Responsibilities:
1. Guided by the interdisciplinary plan of care, gather information related to the patient family physical, psychological, social and spiritual needs, and collaborate with the Registered Nurse, pt./family, physician, and IDG to coordinate care
2. Initiates and maintains communication with Registered Nurse, IDG, Team / Clinical Director, and other agencies to coordinate optimal care and use of resources for the patient/family
3. Documents according to protocol and maintains up-to-date patient records so that the care plan’s problems, interventions, actions and goals reflect baseline, current, and ongoing status, and changes as they occur
4. Participates in Continuous Care, Respite Care, Pre-Hospice or On-Call assignment / rotation as directed. May conduct in Information / Sign-On & Consent visits as assigned
5. In keeping with the plan of care, provides education to patient and caregiver(s) as needed based on knowledge needs and learning assessment and patient / family condition
6. Informs the Registered Nurse, Team Manager/ Clinical Director or AOC and IDG of unusual or potentially problematic patient/family issues
7. Participates in orientation and ongoing education programs and maintains current professional licensure. Demonstrates familiarity and use of agency and state(s) policies and protocols and federal regulations pertaining to provision of hospice and nursing services
8. Participates on committees and special projects and other related duties as assigned
9. May participate in IDG Meetings, facility patient conferences, and other patient / family meetings in coordination with IDG and /or Team Director
10. Other duties as assigned
On-Call
11. May perform routine visits as assigned
12. Coordinates schedule and care activities with Hospice Registered Nurse, IDG, Team/ Clinical Director or AOC and other agencies as necessary
13. Is free to spend time as he or she chooses until he or she becomes engaged in work
Pre-Hospice
14. Under the direction of an APN/RN or physician, assist in execution of the care plan, including routine and emergent visits
15. Contributes to the on-going care planning process, especially regarding medical needs, with the care team and other agencies as required
16. Gathers information regarding patient clinical status as needed utilizing telephonic, telemedicine, and/or face-to-face appointments
17. Provides support for the Pre-Hospice patient and family as new clinical needs are identified or questions arise, with referral to the clinician as appropriate
18. Gathers information regarding the patient for appropriateness for other service lines that will best meet patient needs such as Hospice
19. Collaborate with the rest of the SIP team including alerting the team to changes and newly identified patient needs they arise
Qualifications:
1. Associates degree, preferred
2. Current and valid Licensed Practical Nurse license in state(s) license(s) where practicing required
3. Current and valid CPR certification preferred, required in Georgia
4. Meet the regulations and requirements of the state(s) in which program provides services
5. Minimum one year of experience working in a licensed field, preferred, required in California
6. Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.
7. Must have reliable automobile, valid driver’s license, and the minimum state required liability auto insurance
