At Allay Home & Hospice, we are currently recruiting home care RN’s that possess the skills needed to provide quality care to our patients and their families.
This position is primarily responsible for performing the admission visit for all home health patients referred to service. The Admit nurse must have a good understanding of the OASIS C program, and have good assessment skills that enable them to recognize the need for other disciplines that may be necessary on a case and not yet ordered by the physician. They would be responsible for establishing the initial frequencies and communicate with the physician.
Job Requirements:
At least 2 years of homecare nursing experience is a must.
Current WI state RN license
Travel on a daily basis
Excellent written and verbal communication skills
Ability to work well with an Interdisciplinary team
