Job Profile Summary: A Mental Health Consultant is a full time LICSW provider offering immediate access to mental health care in the Allina Health primary care clinics. As an MHC, you will provide brief solution-focused therapy services and diagnostic assessments while connecting patients to resources. You will combine social work and mental health to support patients of all ages. You will collaborate with primary and specialty care providers in an integrated mental health/primary care clinic setting. Group therapy and longer term treatment are not part of the position.

Principle Responsibilities
- Assessment/reassessment.
- Perform initial clinical assessment.
- Complete a diagnostic assessment that meets the requirements of regulatory agencies.
- Reassess patient and progress in treatment.
- Meet requirements of third party payers, including seeking authorization.
- Treatment Planning.
- Complete treatment plan per department process.
- Documentation of patient, and family if appropriate, participation in development of treatment plan.
- Review treatment plan with patient at regular intervals.
- Initiate referrals to other providers and agencies when appropriate.
- Coordinate discharge planning.
- Provision of Treatment.
- Provide individual, family and group therapy as identified by treatment plan.
- Coordinate treatment with other community agencies/providers when appropriate.
- Provide patient and family education.
- Other duties as assigned.
Job Requirements
- Master’s degree in Mental Health or related field required
- 2 to 5 Years in licensed practice preferred
- Licensed Independent Clinical Social Worker – MN Board of Social Work required upon hire
- BLS Tier 1 – Basic Life Support – Multisource required within 180 Days
