Job Description
Responsibilities:
Primary Job Functions:
- Responsible for completing the Government Performance and Results Act (GPRA) Core Client Outcome Measures on assigned patients at within 30 days of intake and then every 6 months while enrolled in the program.
- Complete discharge GPRA’s when a patient discharges/leaves treatment
- Monitor the GPRA/NOMS report daily to ensure interviews are completed within the allowed time frame given.
- Ensure the information gathered from the GPRA/NOMS is provided to the treatment team when needed/applicable to make sure all needs are being addressed
- Conduct GPRA/NOMS interviews in person utilizing a trauma informed care approach
- Consult with treatment team when a patient has not been seen in clinic as required to complete GPRA/NOMS and staff with clinic manager to ensure appropriate documentation in patient chart to reflect attempt to engage
- Assist patients with connecting to the treatment team as needed
- Assist patients to develop and understand health management plans and goals.
- Coach patients in effective management of their chronic health conditions and self-care
- Assist with removing barriers to treatment
- Assist with improving patient independence
- Motivate patients to be active and engaged participants in their health
- Provide basic living skills as needed
- Work collaboratively and effectively within the treatment team
- Provide emergency care as needed (CPR, MindSet, PRN Medications), according to the AltaPointe Policy & Procedures
- Demonstrate the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
- Seeks clinical supervision and consultation needs
- Accepts and employs suggestions for improvement
- Actively works to enhance clinical skills
- Attends regularly scheduled staff meetings
- Clinical Record Keeping:
- Documents in a timely fashion per AltaPointe policy
- Documents in a clear, concise manner, types of consumer problems, treatment goals and services provided
- Documentation shows that forms, notes, logs, and other applicable written information are completed promptly, accurately and updated when appropriate
- Documents legibly
- Courteous and respectful attitudes towards consumers, visitors and co-workers:
- Treats consumers with care, dignity and compassion
- Respects consumer’s privacy and confidentiality
- Is pleasant and cooperative with others
- Assists consumers and visitors as needed

- Administrative and Other related duties as Assigned:

- Actively participates in AltaPointe committees as required

- Accesses appropriate community resources according to consumer needs
- Follows AltaPointe Policy & Procedures
- Maintains current State license and requirements for renewals (i.e. CEU’s), and attends the required in-services and workshops
Qualifications:
High school diploma/GED required. Any combination of 3 years health/social services experience and/or education preferred. Must meet driving requirments.
