Aviva Health is currently recruiting for a Community Health Program CoordinatorThe Community Health Program Coordinator shall be a core member of the Community Health team. The Program Specialist defines and organizes current Aviva Health work around social determinants, patient complexity, population segmentation, and programming that assesses and responds to these focus areas, as well as serves as the coordinator for the school-based health center and the RH Care Coordinator. This position works closely with internal staff and external organizations and stakeholders to facilitate and encourage the conversations and interventions needed to create health equity for Aviva Health’s patients.QUALIFICATIONS:
- Bilingual Spanish/English strongly preferred
- Experience in management of health care programs which includes supervision of multi-disciplinary teams.
- Experience working with adolescents and underserved populations or communities is strongly preferred.
- Public relations experience
EDUCATIONAL REQUIREMENTS:
- Bachelor’s degree or equivalent work experience in community health education, health promotion, public health, or related field required.
- Master’s in public health preferred
Job Type: Full-timePay: $21.10 – $25.12 per hourBenefits:
