Our behavioral health providers work closely with patients and their Primary Care Providers to help them meet their health care goals.We are seeking a Case Manager for our expanded behavioral health program in Barbour County. Through this position, you will work directly with patients and their families in our Integrated BH program to coordinate care and referrals to wraparound services that support clinical care. The Case Managers make proactive contact to improve patient outcomes, increase patient compliance, improve patient recall and retention, enhance patient advocacy, and develop cooperative relationships with other health and social services agencies.Education/Knowledge/Skills:
Bachelor’s degree in psychology or human services
Experience working in social services and with underserved populations
Experience with Medicaid patients and related insurance knowledgeJob Type: Full-timeBenefits:
