Community Health Outreach WorkerJob Summary: Conduct staff-led outreach, engagement/re-engagement strategies including accompaniment, and follow up calls and visits to participants. Provide outreach, education and referrals to individuals at a variety of venues.ESSENTIAL DUTIES AND RESPONSIBILITIES: Initiate identification and enrollment of eligible clients Syringe exchange and related education Preparation and packaging of outreach supplies Provide education and information on harm reduction, safer sex, infectious disease transmission and prevention, COVID 19 Referrals for primary care, detox, drug treatment, 12 step programs, mental wellness, nutritional services, medical care, and related needs Advocate for participants with providers Document all activities and services provided Participate in Case Conference, team meeting, assigned trainings, and individual supervision Develop and implement outreach strategies Representation of program and participant needs at various community forums and planning bodies, as assigned Conduct syringe waste litter cleanups as scheduled Facilitate Support Groups Other related duties, as assignedRequirements/Criteria: Experience in harm reduction practice Knowledge and experience with outreach strategies and provision Minimum 6 months experience working with active drug users Experience working with people living with HIV/AIDS Knowledge and experience in disease prevention and health education Excellent people skills and oral communication skills Knowledge of social services in NYC Valid NYS driver’s license Bilingual English/Spanish preferred Willing to work flexible hours, including some evenings.*This Agency follows the NYC Workplace Vaccination requirements.We are an Equal Opportunity EmployerPlease visit our website for information: www.boomhealth.orgWe are a Drug-Free Workplace.
