• JOB SUMMARY
  • Works under the supervision of the Ambulatory Clinic Manager and/or Front Desk Supervisor. Maintains Electronic Health Record (EHR) integrity through scanning, filing, copying, assembling, and retrieving patient records. Responsible for the appropriate disbursement of medical records per HIPAA guidelines. Cross trained as Registration Specialist/Cashier.
• PRIMARY JOB DUTIES
  • Is responsible for the integrity of the Electronic Health Record of all patients
  • Performs Scanning and cataloging all documents into patient Electronic Health Record (EHR)
    • Clinic and Provider generated documents
    • Out Patient and ER generated documents
    • In Patient generated documents
    • Pharmacy generated documents
    • Payer generated documents
    • Patient generated documents
    • Pt Employer generated documents
    • Local, State and Federal Agency generated documents
    • Financial documents
    • Other miscellaneous documents
  • Prepares and disburses medical records per HIPAA guidelines
    • Responds promptly and courteously to walk-in and phone requests from patients for copies of health care information
    • Responds verbally, or in writing, to requestors, as appropriate, regarding requests which cannot be processed due to lack of information, misinformation, missing or inadequate release of information form, and delays due to incomplete medical records
    • Ensures the appropriate creation and recording of fees charged, billed, and collected for records and/or forms as requested.
  • Corrects duplicate medical record accounts
  • Performs Storage and/or Destruction of hard copy of scanned items, per HIPAA guidelines
  • Is proficient in Greenway Practice Management applications. Related to position and responsibilities
    • Becomes knowledgeable and proficient in record handling of each clinic, in order to provide coverage for all HRS staff in all clinics
  • Receives, Sorts and distributes clinic mail

  • Maintains confidentiality of all personnel and patient care and relations information
  • Actively participates in Strategic Quality Management for the department and organization. Actively participates in Customer/Guest Relations and Mandatory Education programs
  • Follow hospital and departmental policies and procedures
  • Must be free from governmental sanctions involving health care and/or financial practices.
  • Complies with CCMH Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.
  • Performs other duties as assigned.
• JOB SPECIFICATIONS
  • Education: High school graduate; medical terminology preferred.
  • Licensure: None
  • Certifications required: See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
  • Experience: Previous experience in healthcare field preferred. Experience in a clinic-based medical records department preferred
  • Essential Technical/Motor Skills: Strong computer skills. Computer literate with good keyboarding skills. Eye-hand coordination, manual and finger dexterity. Ability to run a copier. Ability to answer phones. Ability to learn and utilize alphabetical and terminal digit filing systems.
  • Interpersonal Skills: Excellent interaction with patients, coworkers, medical staff, outside agencies effectively and courteously. Strong interpersonal skills. Ability to interact with persons of varying backgrounds and education level. Corresponds effectively with outside agencies. Knowledge of English grammar and spelling
  • Essential Physical Requirements: Ability to bend or stoop, lift, pull, sit for prolonged periods of time, and stand for prolonged periods of time. Ability to lift or carry 10 pounds or less. Ability to reach above shoulder level. Ability to work in small spaces
  • Essential Mental Abilities: Attention to detail. Reasoning. Concentration. Review of requests for appropriate criteria. Ability to interpret and apply federal and state statutes regarding the disclosure of healthcare information
  • Essential Sensory Requirements: Sufficient vision, hearing, and speech to perform duties

  • Hours of Work: Generally, normal business hours (8:00 a.m.-5:00 p.m.); overtime, evenings, and weekends may occasionally be required
  • Ability to work under stressful conditions. May be required to travel to local off-site locations to perform some duties; must have or be able to arrange own transportation
