Job Description:
The New Patient Abstractor abstracts and summarizes new patient charts, serves as a point of contact for moderately complex charts, and working within pre-defined methods, tactics and procedures to impact department goals.
The overall standard is accurate completion of new patient abstracts prior to the initial appointment and serving as a resource with in-depth and broad knowledge on patient charts. The New Patient Abstractor is expected to partner with Intake leadership to understand the department and corporate strategy and operating objectives.
Job Responsibilities
Accurately abstract and document core clinical information from medical records in chronological order that is relevant to the patient’s cancer diagnosis to include diagnostic, treatment and social history. In absence of appropriate documentation, will seek out or query physicians for complete clarification. Promptly communicate missing essential records to the Intake Record Representatives.
Will be assigned a variety of complexity in cancer histories and get direction from physicians or other qualified clinical personnel involving more complex charts.

10% | 2. Maintain working knowledge of cancer and abstracting by attending meetings and educational opportunities suggesting by your leadership. May be asked to assist with training/on-boarding of new Stakeholders.
10% | 3. Support department strategy and key performance indicators by tracking personal productivity/statistics on monthly department log and on Lean Daily Management board. Partners with leadership to understand the department and corporate strategy and operating objectives. Follows established methods, processes and pathways to accomplish goals and seeks out guidance for less routine or new assignments.
10% | 4. Serves as a resource to Nurse Navigation and the Patient Empowerment Care (PEC) team on contents of outside records on incoming new patients.
10% | 5. Supports department strategy and key performance indicators by assisting on projects and assignments as requested by leadership outside of routine work.
