How will this role have an impact?

While there are many goals of the Complex Care Management program, the overarching goal is to improve the quality of life for our members by reducing overall utilization and preventing unnecessary hospitalizations and emergency room visits. Signify Health is focused on improving the quality of life for health plan members through effective care coordination and best in class care management. Our Complex Care Management program services our clients’ dual eligible members residing in long term care facilities. The Complex Care Management RN Care Coordinator role functions within a multidisciplinary team to positively impact member healthcare outcomes.

This role will report to our CCM Clinical Network Manager

What will you do?

• Conduct regular telephonic and/or periodic in-person follow-up visits for members residing in long term care facilities
• Communicate regularly with members, power of attorneys, family members, caregivers, and other medical and facility staff as appropriate to determine member’s identified needs
• Partner with Signify Health Nurse Practitioner, member’s health plan & outside providers, performing care coordination of both internal & external resources to meet member’s identified needs
• Monitor the quality of member’s care, partner with Signify Health Nurse Practitioner and ancillary care team to quickly identify changes in condition or gaps in care and facilitate care coordination and adjustment to the member’s plan of care
• Follow up on currently hospitalized members to determine anticipated date of discharge
• Submit requests for medical records and upload medical records as necessary
• Perform routine and post-hospitalization medication reconciliations
• Perform telephonic outreach to member POAs to discuss program goals and obtain approval for member enrollment
• Provide routine and ongoing education to members, caregivers, and facility staff as needed
• Assist in coordinating appropriate end of life care planning as needed, facilitating palliative care and Hospice discussions with providers, members, and family members
• Engage in client interdisciplinary care team meetings, internal clinical office hours, and other meeting venues as necessary, participating in committees upon request

• Communicate routinely with health plan care managers regarding member status updates
• Assist members/facilities in scheduling appointments with specialists as needed
• Huddle routinely with other members of the Signify Health Care Management team and health plan team as required
• Support and implement organizational technology changes
• Provide backup support for other clinical team members for PTO coverage
• Perform other related duties as responsibilities as assigned

We are looking for someone with:

• Active RN license in good standing without restriction and with ability for multi-state licensing
• Bachelor’s degree preferred
• 2 years Case Management, Utilization Management, or Transitional Care experience
• Case Management certification preferred
• Minimum of 2 years’ clinical experience working with complex adult populations preferred
• Strong knowledge of various chronic medical conditions along with the associated routine lab monitoring, diagnostic, and treatment options as well as commonly prescribed medications for those conditions
• Ability to quickly develop rapport and establish positive working relationships with members, nursing home facility staff and professional colleagues
• Understand and be able to apply principles of Care Management and individualized care planning
• Ability to work in fast-paced environment and adapt well to frequent change
• Strong critical thinking skills
• Strong organizational skills and the ability to prioritize
• Ability to effectively present information and respond to questions from members, providers, team members, and managers
• Multilingual ability a plus
