Overview

Chestnut Family Health Center, a division of Chestnut Health Systems, is seeking a full-time Advanced Practice Registered Nurse for our Granite City, IL facility, servicing the Metro East and St. Louis, MO areas.

Responsibilities

• Provide high-quality, patient-centered, culturally sensitive primary care services:
  • Conduct comprehensive History (HPI)
  • Address Problems and Medications List
  • Conduct thorough Review of Systems (ROS)
  • Perform thorough review of Past Medical History
  • Conduct thorough Physical Examination
  • Develop Assessment and Plan
  • Management of Care / Medical Decision Making
• Provide direct medical prescriber services in accordance with Nurse Practitioner Collaborative Agreement, State of Illinois and agency policies and procedures:
  • Actively participate in Collaboration meetings/discussions
  • Staff cases when appropriate
• Be familiar with and utilize CFHC-approved Clinical Protocols. Actively participate in program development activities to improve services to patients and educate staff on relevant topics.
• Complete by deadline all training assignments.
• Actively participate in multi-disciplinary team staffing and consultation activities including team huddles, meetings and/or calls.
• Perform services diligently and faithfully, in accordance with the Corporate Bylaws and policies of Chestnut or affiliated corporate entities, with the same quality and practice of medicine performed in the community in accordance with all applicable local, state, and federal laws and applicable codes of conduct and the Joint Commission.
• Meet or exceed productivity measures as outlined in the CFHC Provider Expectations document.
• Meet or exceed documentation deadlines as outlined in the CFHC Provider Expectations document.
• Meet or exceed quality measures as outlined in the CFHC Provider Expectations document.
• Assist Chestnut in complying with all applicable standards as required by HRSA, the Joint Commission and other applicable accrediting organizations, licensing bodies and payers.
• Complete financial, credentialing/privileging documentation within the expected timeframes as outlined in the CFHC Provider Expectations Guide and/or policy.
• Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.
• Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and consumer information.
• Perform other duties assigned.

Qualifications

• Must be a Registered Nurse with Master’s degree in Nursing.
• Must be licensed as a Family Nurse Practitioner or other Advanced Practice Registered Nurse in the state of Illinois with a specialization in family practice.
• Must possess a solid understanding of comprehensive primary care services (acute, preventive and chronic care) for pediatric through geriatric patients;
• Must possess experience in primary care or a community health ambulatory setting and experience in provision of chronic care/disease management services.
• Must have a willingness to work in an integrated primary care and behavioral health care environment.
• Must have a willingness to work with underprivileged population.
• Basic computer skills with willingness to use electronic medical records system.
