Job Description
Overview:
Chestnut Family Health Center seeks a Practice Manager to oversee its $5.5 million multi-site health care delivery system blending primary care, behavioral health, addiction treatment, oral health, and prevention services. The position is located in a beautiful setting – space that recently benefitted from an $8 million expansion and renovation.
The Practice Manager will oversee other clinical management positions, provide vision and leadership to ensure high quality care, and ensure that programs and services align with Chestnut’s policies and strategic objectives. The position requires critical thinking, sound judgment, an entrepreneurial approach, and proven problem solving ability.
The successful candidate thrives in a busy, fast-paced environment. He or she will function as part of multiple teams within the agency. Keys to success will include excellent time management skills and the ability to prioritize and balance multiple, diverse projects of significance.
Administrative departments including HR, accounting and finance, management information systems, facilities, purchasing, quality, and risk management all support the Family Health Center and the Practice Manager role.

Responsibilities:
- Provide direction and oversight of a multi-site integrated FQHC health care delivery system blending primary care, behavioral health, addictions, oral health and prevention services.
- Provide direction and oversight of the coordination, integration, assessment and quality outcomes for whole person centered services.
- Evaluate, develop and support CFHC infrastructure that promotes innovation and supports high performance in an environment of rapid health care transformation.
- Provide oversight and direction in the development and implementation of new service lines that align with Chestnut’s mission, vision and values and with community needs.
- Assure program compliance with all HRSA, Joint Commission, HFS, SUPR, DMH and other payer, state and federal requirements.
- Review and approve all clinical and program-specific procedures.
- Demonstrate a commitment to cost-effective systems of care, clinical and management information systems to support ongoing monitoring and decision-making for existing and proposed new services.
- Work closely with VP of Integrated Care and other Chestnut leadership and programs to further strategic goal of regional and cross regional integration and collaboration.
- Maintain relationships with professional associations, community service groups and local and state agencies.
- Participate in corporate management activities including developing corporate policies and procedures, strategic planning, program planning and development, budget planning and development.
- Promote Chestnut’s culture of customer service excellence through the adherence to Chestnut’s behavioral standards for customer service.
Qualifications:
- Position requires a Bachelor’s degree in a healthcare related discipline with at least five (5) years of professional experience in a management position in a medical setting; or a Master’s degree in a healthcare related discipline with at least three (3) years’ experience in a management position in a medical setting OR a minimum of three (3) years’ experience in a leadership position in a Federally Qualified Health Center (FQHC).
- Satisfactory completion of Chestnut’ts background checking process.
