If so, CHI Health at Home is looking for a CNAs to work in Home Health care, where
YOU MATTER
. Where you will work as a Home Health Aide/CNA asisting in daily personal and grooming services, household care and provision of nutrition. This position will work under the supervision of a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Therapist.
Other duties include but not limited too:
Assisting with ambulation, and assists with active or performs passive range motion exercises.
Communicate changes regarding patient’s physical/mental condition, completes and submits documentation in a timely and appropriate manner.
Participates in case conferences, attends in-services, and/or special case orientation.

High school graduate or equivalent.
One year Hospice Aide/CNA experience, patient tech or other related experience and/or passes CHI Health at Home’s competency evaluation program (Medicare regulation)
Must obtain and maintain CPR or BLS certified.
Successful completion of all competency testing.
