Care Coordinator (DDP)

Our Care Coordinator (DDP)s are responsible with maintaining that quality for Comfort of Home Healthcare In-Home care and disability support services. This is done through coordinating our staffing, troubleshooting staffing issues, supervision of all the documentation in our clients’ homes and maintaining or coaching the best practices in the particular home that they are responsible for.
The Care Coordinator (DDP)s is a supervisory position but will also have to work shifts in their homes if the need arises.
Qualifications
5 years of experience in a supervisory role in the healthcare field and/or Bachelors degree.
Compensation and Reporting
Care Coordinator (DDP) is a salary position with a 40-hour work week and directly reporting to Comfort of Home Healthcare Administration.
This is a 24/7 position which does not operate during regular (M-F 8a-5p) hours. Home visits and emergency phone calls will take place partially on evenings, overnights, and weekends.
This is a flex schedule position, all hours including evening, overnight and weekends is to be documented accurately on time sheets and to be turned in weekly.
Supervisory Responsibilities
The Care Coordinator (DDP) is a supervisory role for Comfort of Home Healthcare and is responsible for the daily operation in the homes that they are assigned. Team Leaders and direct care staff report to the Care Coordinator (DDP).
Quality Control Home Visits
The primary responsibility of Care Coordinators (DDP) are monthly home visits to ensure the quality of COHH services, it is the most important functions of this position.
Daily home visits will consist of checking on all documentation so that it is being done correctly in accordance with Missouri Medicaid.
When issues are found, it is the responsibility of the Care Coordinator (DDP) to work with the Team Leader and their staff to address the issues and train the team working in the home.
Staffing Responsibilities
The Care Coordinator (DDP) is to be available to COHH clients, office, and administration 24/7. (When time off is needed, coordinate it with Administration so that all employees are aware and will not call at this time)
The Care Coordinator (DDP) will take the lead/assist the Team Leaders in scheduling and filling open shifts, troubleshoot issues, and help fill call-ins for their house.
The Care Coordinator (DDP) may be called upon to cover shifts him/herself when COHH is short on replacement staff. Care Coordinator’s (DDP) can then adjust their schedules in the next couple days/week to accommodate the extra hours of work.
Other Care Coordinator (DDP) Responsibilities
It is the responsibility of the Care Coordinator (DDP) to maintain quality, professional relationships with the family of COHH clients and their staff as well as any other agency involved such as Hospice, Home Health, or Service Coordination. Professionalism is highly important in this role. Care Coordinator (DDP)s are a direct reflection on the agency.
Care Coordinator (DDP)s are solely responsible for ensuring that all documentation is accurate and complete in their homes. This documentation is to be checked daily by the Team Lead with a follow up check from the Care Coordinator (DDP)
Care Coordinator (DDP)s are responsible for strictly adhering to and coaching of any Care Plans (DMH) or Plan of Care (DHSS) themselves and all staff in the home.
Documentation must be kept and updated daily in order to keep outings and activities in accordance to outcomes in clients care plans.
Care Coordinator (DDP)s are responsible for reporting anything out of the ordinary to the office. Examples are:
Client being injured
Med Errors/ Event Reports
All staffing issues (tardy/call-ins)
Etc.… (If you have questions, ask!)
Care Coordinator (DDP) Job Performance Assessments
All Care Coordinator (DDP)s will initially be placed on a trial basis for 3 months (90 days). After this period, Care Coordinator will meet with COHH Administration to evaluate their job performance. If kept on as a permanent Care Coordinator (DDP), annual job performance evaluations will be conducted thereafter. If Administration does not feel performance is acceptable, either an additional evaluation period will be issued or a determination that the job is not a good match for them and the individual will be placed back as a direct care staff.
Confidentiality
All Care Coordinator (DDP)s are privy to confidential information and its business practices. All of Comfort of Home Healthcare, LLC’s activities are to be treated as confidential. All client and employee information are to be kept confidential even if they are released from their position with Comfort of Home Healthcare. Contact with Comfort of Home Healthcare’s employees and clients if not working for Comfort of Home Healthcare is strictly forbidden.
Training
All Care Coordinator (DDP)s are responsible for coordinating and conducting new hire trainings for both Team Leads, new direct care professionals, and caregivers. Care Coordinator’s will also conduct ongoing staff trainings and lead team meetings when necessary.
Examples of other COHH daily operations and responsibilities
Monthly house checks on their home(s) in the evenings and weekends.
Troubleshoot and assist staffing call in’s and scheduling issues.
Make sure all schedule changes are documented.
Work call in shifts when replacements cannot be found.
Verify that the scheduling system is correct with no errors and that they match the progress notes.
Schedules need completed in scheduling software for the following month by 23rd
Audit and maintain accurate participants’ documentation to ensure they are properly completed in accordance with COHH policy and MMAC compliance.
Keeping guardians contacted and up to date
Make sure all client appointments are made in a timely manner and clients are taken to them. Try to have all appointments scheduled when the Care Coordinator (DDP) is working. Care Coordinator (DDP)s must take to appointments unless approved by Comfort Of Home administration.
Check in all medication at the first of the month and check that medications are being given properly
Attend all plan meetings, service monitoring’s and doctors’ appointments
Participate in pre/post transition meetings
Attend required trainings and webinars
Facilitate new hire and ongoing DCP trainings in the home
Keep track in a planner of all client appointments
Prioritization and balance your time management
Other duties as assigned by Administration.
Make sure ample supply of blank documentation is in the home
Make sure that all documentation is filed in the proper locations in a timely manner
Make sure that EMTs and MED Error reports are filled out and handed in immediately and entered in CIMOR
Make sure all staff are following dress code
Work with Team Leads and Administration monthly with audits
Communicate with Administration weekly for updates and changes
Certifications: (If you have these certifications that is a plus but we will set up training for you if you do not have them) First Aid & CPR, Level 1 Med-Aide Or DDMA, Abuse and Neglect, PBS, EOE/D/V
All employees must pass background check and commit to drug screenings. Must have valid driver’s license, proof of auto insurance, ability to follow instructions, and have excellent time management and multitasking skills.
