Job Description
CHCB is seeking a Psychiatric Nurse Practitioner to join our team at Riverside in Burlington, Vermont. The NP will be responsible for providing direct outpatient mental health services. Renders services that reflect current standards of medical care appropriate to mid-level provider training and experience, and appropriate to local medical community. Coordinates and documents care for quality and continuity of care, and for risk management. Serves as a member of health care team and assumes responsibility for quality assurance and quality improvement efforts.
- Provides direct outpatient mental health services and appropriate follow-up
- Refers patients to appropriate specialty care and communicates/follows-up with specialty providers
- Provides clinical supervision for nursing staff as assigned for triage, medication refills, referrals and nursing services visits
- Provides appropriate education to trainees such as medical students, resident physicians, mid-level students and nursing students
- Participates in overall efforts such as development of flow sheets and data collection instruments, chart audits and case conferences, etc.
- Participates in continuing education appropriate to CHC practice
Basic Qualifications
Education
- Graduation from program of study accredited by the appropriate professional board
Knowledge, Skills, and Abilities
- Ability to work independently as appropriate to level of training/experience
- Ability to work collaboratively with multiple disciplines to support excellent patient care
- Ability to solve problems creatively and with initiative
- Ability to work flexible hours
Licensure, Certification, or Other Requirements
- Board certified or eligible for board certification
- CPR certified
- Licensure for practice in the state of Vermont
- Completion of requirements for CHCB practitioner credentialing
