Where you fit in
The Behavioral Health Consultant will provide support to the physicians and the practice. You will complete diagnostic assessments, develop treatment plans, provide short term, effective evidence based counseling, provide case management, and referral services.

Your ability to collaborate & promote program quality will assist in working with the physicians and their staff. Your flexibility, persistence and patience will help you give your patients the best care.
Your exceptional qualifications
- Masters Degree from an accredited college or university in Social Work.
- 2 + years of experience providing mental health treatment required.
- 2+ years of experience in a physician practice or medical setting preferred
- Valid LCSW, in the State of Indiana required.
