Job Description:
Works closely with the weekend supervisor. Coordinates VNA services by working with the Team Managers, Clinical Staff, Physician’s offices, hospitals and all other referral sources. Updates liaison with hospitalizations of patients as needed. Coordinates home health and hospice services by working with the weekend supervisor and staff. Communicates back to the weekday teams and managers as needed. Receives and coordinates referrals for all programs including: home care, perinatal, IV, rehabilitation, social services, support services, hospice, community palliative care and telehealth. Ensures that referral documents contain F2F components to process referral. Processes workflows for all departments on the weekend related to all stages of a referral including intake, billing and scheduling of the admission timely and with accuracy. Receives and processes SBAR’s, documents per process and communicates as necessary to clinical staff or managers as requested. Triages scheduling calls and field staff concerns. Fulfills additional responsibilities as assigned and as necessary, accurately and in a timely fashion. Participates in personal growth and development. Documents all phone calls in the electronic health record in a timely fashion. Check unassigned list upon starting and when ending shift. Responsible to read weekend report done by Intake on Friday and on Sunday reads Saturday’s report from weekend supervisor to ensure referrals and visits are scheduled. Assigns admission clinicians. Processes scheduling workflow timely and with accuracy including missed visits, urgent next day coordination notes, faxes, weekend reassigned visits, etc. Complete scheduling for next day. Call patients to inform of next day admits. Assists weekend RN with any other tasks as able.
