Inpatient Care Coordinator III – Certified Medical Assistant – REMOTE

Full Time / Days

Monday-Friday 8:00 am to 5:00 pm

Pay Range: $22.04 to $31 DOE

the Inpatient Care Coordinator III – CMA serves as a lead point-of-contact and backup for all other care coordinators. Managing a reduced case load, they will support the inpatient case managers and care coordinators as necessary to ensure seamless training, workflow, and highest level of care.

Job Responsibilities/Duties

• Attend rounds daily and compile a list of action items as assigned by the director
• Assist in lowering bed days by following up on discharge and case closure
• Distribute and follow up on action items appropriately to follow through to completion of tasks
• Special Projects as assigned by the director relating to discharge panning, audits, reduction of bed days, etc.
• Provides coverage for Inpatient Coordinator II when needed
• Error report management from Essette
• Manage the NOMNC tracking- log and audit tracking
• Tracking transfers/ Triage log
• Generate letter for post stabilization and social worker consult/referrals
• Complete LAMC Health Plan Report daily and submit
• Maintains calendars and schedules for After Hours Nurses and On Call Hospitalists monthly
• Assists in faxing of memos to hospitals as needed
• Manage EMR (Electronic Medical Records) for case managers
• Create and maintain Job Aids
• Run daily pending report (external)

Qualifications

Minimum Education:

• High school diploma or equivalent required.

Minimum Experience:

• Minimum one to two (1-2) years in the Inpatient Care Coordinator II role or equivalent required.
• HMO/Medical Group experience preferred.

Req. Certification/Licensure:

• Certified Medical Assistant preferred. Medical Terminology certificate preferred.
