12/3/2021Qualifications/Requirements: * Requires a Bachelor’s Degree or equivalent in a health sciences field, Registered Nurse or Licensed Practical Nurse.Must demonstrate the ability to work with all life cycles in areas of health and lifestyle management. Requires close collaboration with medical providers and community organizations.Individual must be professional, self-motivated and have strong attention to detail, communication, teamwork, customer service and computer skills. Ability to provide bilingual patient care is a plus.Requires driver’s license, automobile insurance and reliable transportation.While performing the duties of this job, the employee is regularly required to talk and hear. This is a moderately active role, which requires the ability to lift and/or move items up to 25 pounds, bending and standing as necessary. Must be able to lift, carry and handle equipment, supplies and other work site materials based on position duty requirements.A cell phone is required for this position. ESRHS will provide a monthly stipend towards cell phone reimbursement.
The Population Served: * The center will provide patient centered primary care of the highest quality to residents, migrants and seasonal farmworkers, as well as visitors of the Eastern Shore of Virginia. Essential Functions 1. Identify and target specific problem areas for patients through screenings and chart review. Establish patient education programs in the center which may include cardiovascular, cancer risk reduction, diabetes, smoking cessation, and weight management. Goal: Full time: 15 new patient appointments per month. Pro-rate for part time.
2. Be a referral source to providers for patients identified as having or at risk for Diabetes, Hyperlipidemia, Asthma/COPD, Obesity, complications of smoking, Hypertension, etc. in order to provide necessary education to improve patient outcomes and promote optimal management of chronic disease. This may include providing brief phone education sessions to patients with abnormal lab results.
3. Establish patient education programs in the community which may include worksite wellness days, school health events, coalition work, community presentations, and community health fairs. Goal: Health Educators will coordinate or attend 10 health related events in the community.
4. Responsible for documentation of patient education in the eClinical Works medical charts. Ensure Health Ed template is used to document face to face and phone visits. Contact all patients that no show Health Educator visits to reschedule. Goal: There should be no more than 3 unlocked notes at any one time.
5. Establish and maintain a library of health education materials. Goal: Information will be organized and work area will be tidy. No expired supplies at the end of the year.
6. Participate in Safe Kids Coalition, distribute safety equipment to community; bicycle helmets, car seats, etc.
7. Must maintain current BLS CPR instruction license and meet yearly requirements for certification. Facilitate the provision of BLS CPR to all required staff. Goal: Required to teach two CPR classes yearly for staff recertification.
8. Participate in quarterly Health Services Staff meetings to improve communication and working relationships among all Eastern Shore Healthcare Organizations and to provide optimum service without duplication of educational efforts provided by other sources.
9. Work with the Pharmacy Assistance Program in assisting patients with accessing the pharmacy indigent medication program.
10. Coordinate Reach Out and Read program at center. Goal: Both required reports will be submitted by due date.
11. Present health education topics at center staff meetings. Goal: Present quarterly.
12. Coordinate with Outreach Worker to access services and provide educational materials for patients. Goal: Attend yearly health educator and outreach meeting.
13. Coordinate VDH car seat distribution program at center. Goal: Submit car seat report each month by due date.
14. Perform no-show counseling as designated at the center level.
15. Assist providers with Medicare Annual Wellness Visits. (Goal: schedule at least 12-15 per week)
16. Review controlled medication agreements with patients as determined by the Controlled Medication Policy, utilizing the template.
17. Perform nursing skills (as determined by individual health educator’s scope of practice) that are deemed necessary for health education visits and/or community health events (see health educator competency).
18. Must demonstrate good internal and external customer service skills.
19. As part of the Patient Centered Medical Home Model of care, the employee will advocate for patient’s best interest and make recommendations for patient special needs such as interpreter services; identified barriers to care such as transportation, medication assistance, outreach, and health education; influence patient’s engagement in their care; promoting a continuum of safe, quality, and cost effective patient care.
20. Employee will abide by the Code of Conduct as documented in the Corporate Compliance Manual.
21. Employee must demonstrate an understanding of the intent, and follow standards and guidelines of the Patient Centered Medical Home and OSHA.22. Must possess the ability to follow supervisor’s instructions and maintain regular attendance.
23. Management may modify, add or remove essential job functions as necessary, or as changing organizational needs require.
