Population Health Navigator Position Description

Under the direction of the Director of Quality Improvement, the Health Navigator is responsible for assisting clients and their families to navigate and access community services, other resources, and adopt healthy behaviors.

Primary Duties:

• Responsible for establishing trusting relationships with clients and their families while providing general support and encouragement.
• Provide ongoing follow-up, education, basic motivational interviewing and goal setting with clients/families.
• Screening clients/families in an effort to assist center providers and clinical staff in identifying barriers to care and working with clients/families to connect them with other community resources as applicable.
• Manages assigned caseload of patients. Provide consistent communication that will allow other health center personnel to evaluate client/family status, ensuring that information and reports provided clearly convey progress toward improving health outcomes. Works closely with health center provider and other personnel to ensure patient receives comprehensive and coordinated care.
• Provide referrals for services to community agencies as appropriate. Assist clients in transportation arrangements and provide appointment reminders under special circumstances.
• Maintains confidentiality of all patient information and adheres to policies/safety practices as established by FCHC.
• Review scheduled appointments and charts post visit for gaps in care.
• Works closely with patients, providers, and vendors to ensure closure of gaps through communication, education, monitoring and documentation, to increase our metric performance, quality-based care incentives and improve the health of our patients.
• Perform other duties as assigned.

• Demonstrates knowledge of community resources available to clients based on needs and/or barriers identified through working with clients and their families.
• Demonstrates understanding of diverse patient population and importance of quality customer service.
• Demonstrates ability to communicate effectively in both written and verbal form.
• Develops and maintains skill level on appropriate computer systems including NextGen, Outlook and Microsoft Office Suite, specifically Word and Excel.
• Ability to manage time effectively.
• Demonstrates the ability to work independently and as part of a team.
• Applies professionalism to all interactions with health center patients, employees, providers, contractors and vendors.
• Demonstrates dependability through good attendance and punctuality.
• Willingness to learn, embrace change and maintain a positive attitude.

Experience, education/training and licensure:

• High school diploma or G.E.D. equivalent.
• Certified/Registered Medical Assistant required.
• 2 years’ experience working in an ambulatory primary care setting preferred.
• Experience working with underserved, vulnerable, and diverse populations.
