Direct Patient Care Responsibilities
- Provide direct assistance with activities of personal care, patient health maintenance, and treatment facilitation under the supervision of a Registered Nurse or Therapist, following physicians’ orders.
- Review and adhere to patient care plans, reporting any issues or necessary revisions to the Case Manager.
- Assist patients with daily living activities such as bathing, dressing, transfers, range of motion exercises, toileting, etc.
- Engage with patients by explaining the care being provided and involving them in the process.
Administrative Duties And Communication
- Document care provided accurately and timely in accordance with Agency policy, including completion of mobile care tasks and mileage logs.
- Collaborate with patients, care providers, family members, and Agency staff by actively listening, responding to patient needs, and communicating relevant information to the Case Manager promptly.
- Perform home maintenance tasks outlined in the care plan, such as house cleaning, dishwashing, laundry, meal preparation, and transportation.

Requirements
- High School Diploma or equivalent.
- Wisconsin CNA certification for work in Federally Certified Home Health Agencies and Hospices.
- Previous experience working with the public.
- Possession of a current/valid WI Driver’s License and access to a reliable/insured vehicle.
