Job Summary
The Community Partnership Coordinator will develop and implement an individual support plan for each worker. This position will maintain all forms and records necessary for the purpose of vocational planning. Individual coaching, job skills development, behavioral observation, and coordination with community resources are pertinent duties in addition to other duties as assigned by the Vocational Services Manager. This position is also responsible for the management of a caseload of Clients, providing vocational rehabilitation services to those individuals who have a barrier to gaining and maintaining employment.Duties and Responsibilities

• Based on referral information, initiate contact with the worker and using available resource create a written support plan for each work.
• Complete all forms (government and agency) required for employment and training, e.g., funding sources, social service, etc.
• Coordinate worker support plan with community agencies involved with the rehabilitation process.
• Write required progress s that are objective, comprehensive and distinct in clarity, keeping in mind the worker’s right to access and understanding of such s.
• Provide one-on-one training to clients to assist them in developing and maintaining the vocational/social skills necessary to retain employment.
• Systematically canvass area employers to identify and develop employment opportunities as well as ongoing and potential business/janitorial contracts.
• Keep the worker’s file up to date – reflecting the progress and provision of the rehabilitation services.
• Participate in all scheduled professional and supervisory meetings.
• Responsible for all workers assigned to him/her including trainees and workers in Extended Employment.
• Maintain adequate and up-to-date paperwork as required by agency policy and funders needs.
• Coordinate rehabilitation plan with work supervisors to insure appropriate service provision.
• Educate and coach individuals towards their person-centered vocational goals and objectives.
• Respect and confidentiality and privacy of each worker assigned.
• Follow rules and regulations as they relate to data privacy, CARF, OSHA, Wage and Hour.
• Speak to business and civic groups whose membership reflects potential job opportunities for people with disabilities.
• Understand all SSI and SSDI employment benefit programs.
• Knowledgeable in all employer incentive programs.
• Provide pre-employment (development) and post-employment (coaching) support services as needed and directed.
• Participate on in-house committees as assigned by the Vocational Services Manager.
• Be effective and professional representing the agency in all interactions with public, as well as in public relations work, e.g., membership in civic affairs, public speaking; Goodwill tours; development of self professionally through supervision, professional readings, membership in professional organizations, attendance in training programs.
• Assign tasks to workers.
• Demonstrate quality/quantity criteria.
• Initiate and follow all applicable safety procedures.
• Accept other job-related duties as assigned.

Required Skills and Abilities

• Ability to relate effectively with workers, referring agent and community resource people.
• Ability to speak and write clearly.
• Knowledge of the service delivery system as it affects the worker.
• Adequate computer knowledge to perform all job duties.
• Valid driver’s license, current automobile insurance, and acceptable driving record.

Education and Experience

• B.A./B.S. degree in related field preferred (hours of work in the field can compensate for education)
• Ability to successfully pass a background check.
