Grace Community Home Health is a Christian/Faith-Based Agency seeking a per-diem Speech Language Pathologist (SLP) who has a desire to provide excellent care, and wants to be part of a team who loves to serve our community.Speech Language Pathologist Home Health Position Summary

• Full independence and schedule flexibility
• Independently evaluate and conduct follow-up visits as a Speech Therapist
• Provide Speech Therapy services to clients in their home
• Document patient assessment and visit details in EHR and notify Clinical Supervisor of findings
• Communicate care and updates with the care team
• Submit documentation of Evaluation and Follow up visits according to the company policies
• Evaluate and create plan of care for patients independently

Qualifications

• Friendly, cheerful, and excellent customer service/bedside manners
• Minimum of 1-year experience seeing patients in the home setting (preferred)
• 2 years as a Speech Therapist in an acute or rehab facility
• Active California Speech Therapy License
