Hybrid Schedule-Hybrid is defined as a flexible work model that supports a blend of in the physical office (1-2 days per week) and partially remote workplace.

The Care Coordination Program (CCP) at HCAM is an outreach program that provides short-term care coordination services to Baltimore City Residents. The Care Coordination Program assists Medicaid/HealthChoice eligible beneficiaries in accessing, using, and maximizing their Medicaid benefits. The CCP multidisciplinary team educates clients about health benefits, link residents to primary care and specialty services, work to improve birth outcomes in Baltimore City with innovative prenatal outreach strategies and strive to protect our city’s most vulnerable populations by providing access to health services and community resources.

Annually more than 500 Medicaid/Medicaid-eligible pregnant women become disengaged and/or never engage with the healthcare system increasing their likelihood of poor pregnancy and birth outcomes. B’More for Healthy Babies (BHB) envisions that All Baltimore children are born healthy, grow, and thrive in healthy families. This is done with the improvement of health, quality care during pregnancy, supporting families and promoting social and economic justice.

The BHB Community Health Worker will be responsible for promoting in the community the benefits of self-referring into Care Coordination and the B’more for Healthy Babies Centralized Intake System. This individual will work closely with a marketing team to generate greater understanding and name recognition of B’More for Healthy Babies with the goal of increasing the number of self-referred, unduplicated clients. The BHB CHW will assist interested pregnant and postpartum families to access medical, dental, and mental health services, ensure the enrollment into community resources, parenting support activities and resolve Social Determinants of health.

This position is classified as a hybrid. The hybrid job position requires this staffer to work from home, in the community and at partner sites as well, and be available to report to HCAM Headquarters as needed.

Essential Duties

• Work collaboratively with community-based organizations to provide education about the BHB Centralized Intake system and the use of the electronic Self- Referral process.
• Document BHB Awareness activities and track self-referrals to the program.
• Complete face-to-face outreach with clients in the community or telephonically to assess, refer and confirm enrollment into services including connection to primary/specialty care, MCO Case Management, B’More for Healthy Babies Centralized Intake Services (Home visiting, Safe Sleep, WIC, etc.).
• Conduct a Social Determinants of Health assessment to identify health-related social needs, including housing, utility assistance, food access, and transportation needs. Aid with the removal of barriers to accessing healthcare and other benefited services.
• Documentation of all outreach activities according to standard operating procedures in the electronic medical record, eClinicalWorks database.
• Attend and participate in city-wide BHB community and partnership meetings, local initiatives, events, and meetings promoting the B’More for Healthy Babies services.
• Participate in professional development opportunities that build effectiveness in areas of client engagement and intervention, motivational interviewing, trauma-informed outreach strategies.

Qualifications

• Bachelor’s Degree in a health-related area preferred.
• Certification in Community Health Work is a bonus.
• At least two years of experience providing home/community outreach to the maternal and child health population in Baltimore City.
• Demonstrated knowledge in the following areas: Community Outreach and Medicaid System Navigation.
• Demonstrated knowledge and experience working with HealthChoice Medicaid recipients and Managed Care Organizations.
• Bilingual in Spanish is a bonus but not required.

• Strong computer literacy skills, specifically with MS Word, Excel, Outlook, and SharePoint (365).
• Organizational skills with the ability to meet a demanding workload.
• Excellent oral and written interpersonal and communication skills required.
• Organizational and Critical thinking skills
• Proven ability to work efficiently and meet deadlines.
• Strong knowledge of community resources
• Ability to work independently and as part of a team to achieve project success in outreaching clients identified for care coordination.
