Hybrid Schedule-Hybrid is defined as a flexible work model that supports a blend of in the physical office (1-2 days per week) and partially remote workplace.
The Care Coordination Program (CCP) at HCAM is an outreach program that provides short-term care coordination services to Baltimore City Residents. The Care Coordination Program assists Medicaid/HealthChoice eligible beneficiaries in accessing, using, and maximizing their Medicaid benefits. The CCP multidisciplinary team educates clients about health benefits, link residents to primary care and specialty services, work to improve birth outcomes in Baltimore City with innovative prenatal outreach strategies and strive to protect our city’s most vulnerable populations by providing access to health services and community resources.
Annually more than 500 Medicaid/Medicaid-eligible pregnant women become disengaged and/or never engage with the healthcare system increasing their likelihood of poor pregnancy and birth outcomes. B’More for Healthy Babies (BHB) envisions that All Baltimore children are born healthy, grow, and thrive in healthy families. This is done with the improvement of health, quality care during pregnancy, supporting families and promoting social and economic justice.
The BHB Community Health Worker will be responsible for promoting in the community the benefits of self-referring into Care Coordination and the B’more for Healthy Babies Centralized Intake System. This individual will work closely with a marketing team to generate greater understanding and name recognition of B’More for Healthy Babies with the goal of increasing the number of self-referred, unduplicated clients. The BHB CHW will assist interested pregnant and postpartum families to access medical, dental, and mental health services, ensure the enrollment into community resources, parenting support activities and resolve Social Determinants of health.
This position is classified as a hybrid. The hybrid job position requires this staffer to work from home, in the community and at partner sites as well, and be available to report to HCAM Headquarters as needed.
