Job Description/Essential Functions:
Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

The Certified Registered Nurse Practitioner (CRNP) will provide clinical assessments, evaluations, medication management, psychiatric treatment and crisis assessment/intervention. Facilitate the integration of behavioral and physical health services and promote person centered health and well beingand provide 24/7 crisis response treatment that may include the use of telepsychiatry.The successful candidate’s responsibilities include the following:

*Conduct initial and follow-up evaluations of participants. Evaluations include a psychiatric assessment and recommendation for specific services.
*Provide direct psychiatric services through the comprehensive evaluation, diagnosis, planning and treatment of Crisis Residential participants.
*Order and interpret diagnostic and therapeutic tests relative to the participant’s specific needs. Maintain a collaborative relationship with the multidisciplinary team of professionals.
*Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities.
*Facilitate the integration of behavioral and physical health by linking participants with primary care services and following recommended treatments and referrals to specialists.
Job Qualifications:
To be a successful applicant, one must have the following:
*Minimum of a Master’s degree in Nursing from an accredited education institution required; Current Psychiatric/Mental Health Nurse Practitioner certification or a Family Nurse Practitioner Certification with a Post Master’s Certificate in Psychiatric Mental Health required; Current unencumbered CRN licensure in the state of Pennsylvania.
*Minimum of five (5) years of clinical psychiatric experience with two (2) years’ experience as a Nurse Practitioner.
*Strong computer skills including, Microsoft Word, Excel, PowerPoint, and electronic health records systems.
*Familiarity with and comfort in using telepsychiatry to provide treatment.
*Valid driver’s license with an acceptable driving record required.

Scope:
* Requires ability to sit for long periods of time.
* Requires flexibility in scheduling and ability to do 24-hour on-call coverage.
* Requires ability to lift, carry, fold, kneel, reach, stack, stoop and bend.
* May be required to report to work during emergencies including inclement weather.
* Light lifting and/or carrying, not to exceed 25 lbs.
