The Idaho Department of Health & Welfare is currently accepting applications for a Residential Assisted Living Facility Surveyor position. The role of the surveyor is to conduct surveys and investigations, and apply state licensure laws and rules to ensure the residents of Idaho Residential Care Assisted Living Facilities (RALFs) receive quality care in a safe, humane, home-like living environment where their rights are protected.We are seeking applicants who have at least one year of experience working in Assisted Living, and/or certification/licensure listed under Minimum Qualifications. Previous health facility surveying experience is a plus.Note: These positions are located in Boise only. Incumbents in these positions travel extensively throughout the state leaving Monday and returning Friday on travel weeks, which are typically every other week throughout the year. All positions require travel. • Conduct outcome-oriented, resident-focused surveys and investigations
• Assess each resident’s health, social, emotional and habilitative needs
• Observe care and services being providing to residents
• Observe overall experience of individual as a resident of the facility, including expression of their rights, meals and nutrition, activities and social interactions, cleanliness, maintenance and comfort of living environment
• Conduct interviews and determine through professional judgment if the facility is meeting the residents’ needs in accordance with state regulations and health care standards
• Use professional, collaborative approach to communicate survey findings and provide technical assistance to facility managers
• Document observations, interviews and record reviews in surveyor notes throughout the survey process
• Write survey findings and update electronic licensure records using multiple software applications and desktop, laptop and tablet equipment
• Conduct trainings for industry representatives and assist in development of on-line training courses, newsletters, best practice forms and other tools

Minimum Qualifications:

• At least one year’s experience providing medical care in a health care delivery system, (e.g., an institution, clinic, or office setting, either military or civilian)
• Possess a valid driver’s license
• This position requires one of the following:

• Certification as a Registered Dietitian by the Academy of Nutrition and Dietetics. For licensure information, please contact the Idaho Board of Medicine at http://bom.idaho.gov/
• Licensure as a Social Worker in accordance with Idaho Code 54-32. For licensure information, please contact the Idaho Occupational License Bureau at http://ibol.idaho.gov/
• Qualified as an Intellectual Disabilities Professional (QIDP or QMRP)
• Licensure as a Registered Nurse in Idaho or state licensure as a Registered Nurse pursuant to the Nurse Licensure Compact, Idaho Code 54-1418. For licensure information, please contact the Idaho Board of Nursing at www.ibn.idaho.gov.
• Licensure as a Residential Care Administrator by the Idaho Bureau of Occupational licensing at http://www.ibol.idaho.gov/
• Licensed Physical Therapist by the Idaho Bureau of Occupational licensing at http://www.ibol.idaho.gov/
• Licensure as an Occupational Therapist in Idaho in accordance with Idaho Code 54-3703. Licensure or a limited permit must be obtained prior to appointment
