The Community Health Worker activities will include screening and enrollment for JHHC programs; patient mentorship; patient education; patient follow-up and reinforcement of treatment plan tasks; follow-up on Value Based Purchasing (VBP) Postpartum measure; coordination of appointments and transportation; and patient location and engagement. The CHW must foster cohesive relationships not only with members in the clinics but also with community partners and medical teams. This position will work with an interdisciplinary team of physicians, nurses, Case Managers, Social Workers, Dieticians and other community health workers to improve health outcomes. CHW will be active in community engagement activities in the community, to include working with community organizations, knowing community members and their health issues, assisting with community activities. May direct the tasks of volunteers. CHW must have reliable daily transportation for work related travel (to and from community site, patient homes, meetings, clinics, etc.). If using personal vehicle for work related travel, CHW must have appropriate license and insurance.
The CHW will receive clinical oversight from one or more Case Managers, which includes assignment of tasks. Under minimal supervision addresses the complex social and health problems of patients enrolled in programs. CHW II may be assigned more challenging patients than CHW I. Activities include patient location and engagement, coordination of appointments and transportation, patient mentorship, reinforcement of treatment plan tasks, patient education, visible and active presence within the community for assessment and education, and all activities and functions directed towards supporting patients in obtaining medical treatment and preventive health care services. Acts as mentor and assists with training for CHW Level I, may direct some work of CHW Level I. Leads special projects as assigned by the PM.
Job Requirements:
Education:

• High school diploma or equivalent required. Must be able to speak, read and write in English.

Knowledge:

• Requires working knowledge of social and health issues. Familiarity with medical terminology beneficial.
• Additionally familiarity with resources in East Baltimore related to health and social
  stabilization strongly preferred.

Skills:

• Must have neighborhood leadership and relational skills.
• Must be responsible and well-respected in the community.
• Work requires excellent interpersonal and communication skills to efficiently work with diverse groups of community members and to coordinate with team members.
• Work requires some critical thinking skills to effectively assess social/health needs of members and to arrange for appropriate services required to meet those needs.
• Must be able to recognize situations that require professional intervention.
• Work requires excellent organizational skills, flexibility, and the ability to get things done.
• Must be compassionate and sensitive to diverse situations, results-oriented, a self-starter and capable of making appropriate decisions independently.

Required Licensure:

• None

Work Experience:

• Work requires emphasis on social community or health care related disciplines as acquired through two years of work experience.
• Beginning experience in personnel supervision is preferred.

Working Conditions:

• Telecommuter. Significant portion of work is done at home. Must provide an appropriate space to work consistently at home. Must have a home space that ensures privacy of member information in all forms such as verbal, hard copy, and electronic. CHW II will supply a high-speed internet connection that meets JHHC IS standards. Must have a file cabinet with a working lock to secure records and JHHC equipment.
• Frequently works in the community and in community facilities such as clinics or community centers. Work requires extensive travel within the local community.
• Visits to members’ homes involve exposure to environments outside of the control of JHHC (e.g., dirt, noise, crime, etc.).
• A limited amount of work will be done from an office or clinic environment that meets the minimum safety, health and ADA standards
