The Community Health Worker activities will include screening and enrollment for JHHC programs; patient mentorship; patient education; patient follow-up and reinforcement of treatment plan tasks; follow-up on Value Based Purchasing (VBP) Postpartum measure; coordination of appointments and transportation; and patient location and engagement. The CHW must foster cohesive relationships not only with members in the clinics but also with community partners and medical teams. This position will work with an interdisciplinary team of physicians, nurses, Case Managers, Social Workers, Dieticians and other community health workers to improve health outcomes. CHW will be active in community engagement activities in the community, to include working with community organizations, knowing community members and their health issues, assisting with community activities. May direct the tasks of volunteers. CHW must have reliable daily transportation for work related travel (to and from community site, patient homes, meetings, clinics, etc.). If using personal vehicle for work related travel, CHW must have appropriate license and insurance.
The CHW will receive clinical oversight from one or more Case Managers, which includes assignment of tasks. Under minimal supervision addresses the complex social and health problems of patients enrolled in programs. CHW II may be assigned more challenging patients than CHW I. Activities include patient location and engagement, coordination of appointments and transportation, patient mentorship, reinforcement of treatment plan tasks, patient education, visible and active presence within the community for assessment and education, and all activities and functions directed towards supporting patients in obtaining medical treatment and preventive health care services. Acts as mentor and assists with training for CHW Level I, may direct some work of CHW Level I. Leads special projects as assigned by the PM.
