FOLLOWING IS ADDITIONAL INFORMATION:
The Quartz Care Coordinator reports directly to the assigned supervisor with overall program supervision provided by Clinical Team Manager. Minimum annual service hours are 1352.  This position is included in the professional bargaining unit. 
QUALIFICATIONS
ESSENTIAL
Education/Licensure 

• Master’s degree in a related behavioral health field.
• Licensed as a mental health professional; Clinical Social Worker (LCSW), Professional Counselor (LPC) or Marriage and Family Therapist (LMFT) required.

Experience

• 1 year of experience in related behavioral health field.
• Understand and demonstrate essential elements of cultural sensitivity.
• Strong commitment to principles of recovery and to facilitating consumer participation in treatment/service planning and provision.
• Knowledge and/or experience with the use of evidence-based practices for treatment of substance use disorders and co-occurring disorders.
• Experience in community-based mental health and co-occurring assessments, case management, treatment planning and crisis intervention.
• Excellent verbal and written communication skills with both internal and external customers.
• Ability to work effectively with an interdisciplinary team with a positive and creative attitude.
• Ability to work independently and remain organized and efficient.
• Superior ethics and interpersonal boundaries as evidenced by experience in community-based work with related professional references.
• Possession of a valid driver’s license, a favorable driving record, adequate insurance (per WI law) and access to a vehicle with willingness to use it for work
• Proficient in the use of Microsoft Office Suite applications

Typical Physical Demands
Requires driving, sitting, standing, light lifting, bending, and reaching; and the ability to use and operate standard office equipment, including computer, calculator, phone, printer, cell phone, facsimile, etc.
Working Conditions
May require evening or weekend hours depending on program and consumer’s needs. 
Proof of COVID-19 vaccination, or submission of ongoing PCR test results required.
PREFERRED

• Bi-lingual, and/or bi-cultural (Spanish/Hmong).
• Fluent in American Sign Language. 

RESPONSIBILITIES:
A. Provision of Clinical Services

1. Performs accurate assessments to evaluate and diagnose mental health, substance use and/or co-occurring issues in a culturally sensitive, recovery oriented, trauma informed manner by providing assessments within the context of the consumer’s perspectives and beliefs.
2. Provides a range of clinical services including level of care assessment, service coordination, therapy/counseling, support, and referrals in a context that is supportive of the consumer’s culture, values and perspectives.
3. Provide appropriate level of interim clinical services to help bridge those consumers who may require a higher level of care until those services can be accommodated.
4. Provides care coordination or case management for members between time of intake and the time they access their initial therapy appointment
5. Coordinates production and communication of reports required by Quartz regarding access timeliness, clinical quality measures, referrals, etc. This may include reporting required by OCI related to member satisfaction.
6. Completes accurate clinical records including intakes, assessments, treatment plans and updates, progress notes, discharge summaries and other information in a timely manner.
7. Works collaboratively with other community agencies to develop and maintain a continuum of care and maintain efficient use of community services.
8. Provides consultation and support related to areas of specialization.
9. Attends JMHC Team Meetings and presents consumer cultural and clinical information.
10. Attends monthly workgroup and quarterly Behavioral Healthcare Quality Committee Quartz meetings to present reports required by the HMO, JMHC organizational updates and engage in discussions to improve the experience for Quartz BC+ members and help remove barriers to care. This will include routine reports to the BC+ Oversight Committee. i.e. monthly access reports with requested detail should be available to Quartz within 10 days following the end of each month
11. Attends meetings with primary contact at Quartz and others on an as needed basis to build relationship and address questions, concerns, or quality issues as they arise
12. Maintains minimum direct and case management service hour expectations.
13. Works collaboratively with team members and takes a leadership role in service planning.
14. Work cooperatively with Quartz on quality improvement initiatives, HEDIS (Healthcare Effectiveness Data and Information Set) measure improvement by conducting outreach for specific behavioral health measures to achieve 75th percentile results, NCQA (National Committee for Quality Assurance) compliance, and DHS Performance Improvement Projects (PIP). 

B. Program and JMHC Participation

1. Attends staff meetings, in-services and other program activities as requested.
2. Participates in JMHC committees, trainings and activities as directed and/or approved.
3. Provides consultation to other JMHC programs as necessary.
