KPG Healthcare is currently seeking Medical Case Workers fill and Local Contract opportunities through our network of healthcare facilities.
About the assignment:
Medical Case Worker works in a jail-based setting. Usually meets with the inmate twice to conduct an information-gathering intake to pinpoint what type of resources he/she may need once theyxe2x80x99re released. Resources may include; housing, referrals to mental health or substance abuse agencies/facilities, food stamps, MediCal, etc. MCW will create a re-entry plan for him/her. Rest of the job involves documentation, follow-ups, etc. The CHW continues to follow up with the individual after theyxe2x80x99re released.
This program focuses on assisting three populations: those that are homeless or going to be homeless when released, and those experiencing mental health illness/disorders and/or those challenged by substance abuse issues.
Ideally looking for someone who is comfortable working in this arena and is passionate about helping these populations; someone that is computer savvy, gets along with others in the workplace, is trainable, quick learner, and has soft skills to empathize with the inmate when conducting intakes.

xc2xb7 Education: A Bachelor’s Degree from an accredited College or University
xc2xb7 Shift Time: Variable Schedules avail pending on need
xc2xb7 Weekly Hours: 40hrs Per Week; some schedule requires either evening or weekends.
xc2xb7 Assignment Length: On Going
xc2xb7 Ability to commit to full-time hours, with possible evening or weekend shifts.
