REGISTERED NURSE (RN) – HOME CARE VISIT NURSE FULL TIME DAYS
$15,000 SIGN ON BONUS (sign on bonus prorated based on FTE)
Job Summary:
Performs comprehensive subjective and objective ongoing assessments. Follows an established individualized plan of care under the direction of the RN Care Manager and in accordance with physician orders. Performs skilled treatments and interventions and provides patient education aimed at achieving patient goals/outcomes.
Education – Required:
- Specialized Diploma Nursing
Education – Preferred:
- Bachelor’s Degree Nursing
Experience – Required:
- 1 year recent experience providing direct care in a healthcare setting
Experience – Preferred:
- 1 year Home Health experience
Knowledge, Skills, and Abilities – Required:
- Knowledge of patient education techniques and principles
- Ability to take initiative and function independently without direct supervision and as a member of the care team
- Ability to travel to patient care assignments
- Knowledge of Home Health Conditions of Participation
Knowledge, Skills, and Abilities – Preferred:
- Knowledge of and skill in completing OASIS data set collection with a high level of accuracy
Licensure – Required:
- Registered Nurse active unrestricted PA license Upon Hire
- Drivers License valid in PA Upon Hire
Certifications – Required:
- CPR – Cardiopulmonary Resuscitation BLS Healthcare Provider (American Heart Association) Upon Hire
Licensure – Preferred:
- CACRN – Certified Ambulatory Care RN within 2 Years or
- Clinical Area of Specialty Certification med/surg, oncology, geriatrics, case management Upon Hire
Additional Qualifications:
- Proof of car insurance at PA State minimum requirements – Home Health Access to a reliable automobile – Home Health
