Job Description

LIV HEALTH is looking for a smart, motivated, community-minded, and patient-centered individual to provide part-time community caregiver support to individuals in their homes and community facilities. This position works under our LIV More and Chronic Care Management Program and provides transportation, assists individuals with remote monitoring devices and technology assistance, meal preparation, check-in visits, errands, homemaker services, and other supportive services. May require some weekends but no overnights. Shift differential pay for weekends and holidays. This position works closely with our case management and medical teams. Our goal is to grow this position into a full-time position.
