In the role of Community Health Worker I you will work as part of the clinical team in a Family Medicine Clinic that is focused on the treatment of substance use disorders.The primary responsibility of the Peer Navigator will be to provide support and guidance to patients and their families who are struggling with addiction.The Peer Navigator will work closely with the clinical team to identify and address barriers to care, as well as to provide ongoing support and advocacy for patients.
