Job Summary
This position will provide centralized oversight and is responsible for the administration and management of both ER Navigation and Community Outreach programs and initiatives within Community Health. This position is responsible for working closely with Memorial Hermann and Community Partners to develop and enhance all CHW programs to ensure all program goals are met.

The incumbent will support the Director with administrative functions of these programs, including time management, budget development, administration, and staffing. This position will provide support and guidance to staff in identifying and connecting uninsured and underinsured populations with community health and social service resources and benefits. This position will require the incumbent to have extensive knowledge and understanding of issues specific to access of healthcare by uninsured and underinsured populations. This position will require travel.

Minimum Qualifications

Education: Associates in Health Services, Public Health, Sociology, Psychology, or related field required; Bachelor’s degree preferred

Licenses/Certifications: Certified Community Health Worker required

Experience / Knowledge / Skills:

• In lieu of degree, five (5) years experience working in a community or healthcare setting required
• At least two (2) years in a leadership position with progressive responsibilities preferred
• Knows and understands issues specific to access of healthcare by uninsured and under-insured populations
• Advanced computer skills
• Must have ability to perform complex and diverse administrative duties that involve application of procedures, independent analysis, interpretation of data and demonstrated appropriate judgment
• Effective oral and written communication skills

Principal Accountabilities

• Manages staff for department programs as it relates to recruitment, hiring, performance reviews, and budget management, including quality of interventions and outcomes, volumes, and penetration of hospitals and community settings.
• Ensures CHW workers are providing ER Navigator coverage for Memorial Hermann’s emergency rooms and community sites and monitors that coverage is commensurate with need.
• Provides oversight and guidance to the development and maintenance of the department focus areas including: care coordination, community outreach and quality improvement.
• Strategically develops and implements high impact community health initiatives and projects to advance the goals of the CHW Programs/Initiatives.
• Engages in design, monitoring, and outcome reporting at all stages of an initiative/project lifecycle (concept, assessment, product development, pilot, implementation, training, dissemination, and evaluation).
• Maintains appropriate CHW Program initiative plans, revising the plan periodically and reporting progress towards plan goals to multiple stakeholder groups, including institution senior leadership, funders and collaborators.
• Develops plan evaluation metrics and methods and utilizes these to monitor and report progress.
• Identifies and develops collaborative relationships, Sources resources through partners who share CHW Program goals.
• Develops, manages, and maintains positive relationships with internal and external stakeholders at hospitals and local organizations with shared interests in community health workers and project initiatives.
• Engages as a leader or member of health coalitions or health associations at local levels.
• Identifies and develops strategic partnerships and cultivates relationships with key stakeholders in chronic disease prevention and social determinants, including community leaders and not-for-profit and funder organizations to advance CHW Programs’ goals.
• Generates assigned projects monthly, quarterly and annually. Reports and monitors milestones on an ongoing basis.
• Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
• Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
