Mindoula is seeking a Program Manager in our Population Health Program to lead a team of Case Managers within Kentucky.

Our Case Managers empower members to actively participate in improving their own healthcare through personalized case management that targets each member’s specific needs. An innovative Program Manager seeks creative solutions to support Case Managers and Population Health initiatives, aligning with program goals and driving toward the desired outcomes. As the leader of a community-based team, they model a hands-on approach, empowering the team to meet members where they are, and tapping into the wide range of local resources to deliver whole-person care.
Location: Remote from home, must live within Kentucky
Expectations: Interview time commitment would four hours total (meeting with our people team, leadership and peers)
