The MUSC Heart Failure Care Coordinator reports to the Administrator of Clinical Transformation for the MUSC Population Health Program, and works closely with both the Population Health, and Heart Failure and Heart Failure Clinic leadership teams and staff. The Care Coordinator will facilitate coordination and transition of patient care. The Care Coordinator will assist in the identification of individuals discharged with heart failure who are at high risk for readmission and would benefit from management of heart failure from the dedicated heart failure team. The Care Coordinator will assist in data collection for monitoring outcomes, trends, and 30-day readmissions, and assist in identifying and overcoming barriers to care. The Care Coordinator serves as a liaison between patients, physicians, hospital staff, and outpatient clinic staff. The Care Coordinator will facilitate coordination and transition of care to the outpatient clinic and ensure follow-up occurs. This position works closely with both the Inpatient and Outpatient Heart Failure Navigators and providers along the continuum of care for Heart Failure patients.

Education: Bachelor’s degree in Nursing required. Four (4) years of related work experience as a registered nurse required. Case Management experience preferred. Ensure the provision of nursing care services in collaboration with other health professionals. Current knowledge base of procedures, techniques, equipment, technical advances and regulatory agencies and standards within the field of nursing. Current South Carolina licensure or licensure in a compact state as a registered nurse required. Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider.
