NATIVE HEALTH’s Community Health and Wellness Department educates and engages members of the community about health issues and healthier lifestyles. The goal is to present timely and relevant information, education, resources, programs and tools to help individuals and families enjoy good health now and for years to come.

The Community Health Worker provides culturally relevant education on prevention and the promotion of holistic wellness for Indigenous peoples residing in Maricopa County. Additionally, is a liaison between, the NATIVE HEALTH Community Health and Wellness Department, Medical Department, and community members to facilitate access to services and improve the quality-of-service delivery. Including the coordination of services to improve diabetes-related health outcomes. The CHW reports directly to the Indigenous Wellness Manager.

Qualifications
QUALIFICATION/EXPERIENCE/REQUIREMENTS:

• A Bachelor’s Degree in a health-related field is required.High School graduate or GED equivalency.
• Experience in community outreach or health education is a plus.
• Knowledge of Electronic Health Records (EHR) preferred, NEXTGEN a plus.
• Must be able to work irregular hours to include evenings and weekends as needed.
• Must possess two years of specialized experience working within the American Indian/Alaska Native community.
• Must have a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out of state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid AZ driver’s license during employment. Must be insurable by NATIVE HEALTH liability auto policy.
• Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) days of employment.
• Must pass substance abuse testing upon employment and submit to random testing during employment.
• Must have an updated Immunization (IZ) Record.
• Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an update annually.
• Must obtain a CPR card within ninety (90) calendar days of employment and maintain a valid CPR card during employment.

KNOWLEDGE, SKILLS, AND ABILITIES:

• Facilitate group classes proficiently using a wide range of skills and techniques to help participants understand their common objectives and assist them to achieve them.
• Understand how social determinants of health impact the community and participants.
• Knowledge of Arizona Indigenous tribes’ practices of farming, gardening, and traditional foods.
• Knowledge of client confidentially and client rights according to Health Insurance Portability and Accountability Act (HIPAA).
• Ability to pass blood pressure and blood glucose testing competencies.
• Strong problem-solving skills and assertive in challenging situations.
• Highly resourceful team-player, with the ability to also be effective independently.
• Creative thinker, who actively seeks opportunities and proposes solutions.
• Demonstrates ability to write clear and concise reports of service activities and other programmatic reports.
• Strong interpersonal skills including tact and flexibility to work effectively with supervisor, staff, and participants.
• Advanced computer skills included MS office, specifically EXCEL, PowerPoint, Outlook, and Word.
• Develop creative ideas, manage, and deliver diabetes-related education via digital content using a range of techniques, across channels including websites, social media, and Zoom platform.
• Culturally competent; able to work with a diverse workforce and client population.
