  • Responsible for managing Medical Record completion via the Physician Deficiency Policy and Procedure and completion of transition of care documents via Direct Messaging.
• Job Requirements

  Education

  • High school diploma, General Educational Development (GED) or equivalent required.
  • Completion of medical terminology coursework required.

  Experience

  • Two years prior experience in a Health Information or record management role required.

  License/Certifications

  • Requires the ability to provide transportation to other campus locations within a reasonable timeframe.

Essential Job Functions

Essential Functions I

• Provide reception area and administrative support in order to ensure effective and efficient department operations.
  • Greets guests with courtesy and promotes a positive image of the department.
  • Answer phone calls within 3 rings 100% of the time.
  • Records messages accurately and fulfills request within 15 minutes of request 95% of the time.
• Produces copies of medical records for correspondence requests, physicians, health care facilities and other authorized parties.
  • Verifies that any release of information is consistent with Health Insurance Portability and Accountability Act (HIPAA) applicable federal and state laws concerning confidentiality and privacy.
  • Reviews records for completeness and supporting documentation.
  • Upon receipt of a request for information accompanied by an adequate authorization from the patient or legal guardian, information will be sent to the requestor in a timely manner and within all hospital, Health Insurance Portability and Accountability Act (HIPAA), Federal, State, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) regulations.
  • Request to be completed within 30 days of receipt of request 100% of the time if large medical record request.
• Maintains disclosure log by entering information into the correspondence management system in keeping with HIPAA legislation.
  • Accounts for 40 disclosures per hour worked.
• Assist as back up in the imaging area for scanning.
  • Look up MRN and FIN for stray records.
  • Responsible for scanning documents that are received during the Physician Deficiency Process (Signed orders).
• Participate in the management and completion of the physician orders.
  • Prepare and fax out delinquent records for physician completion and follow up to assure completion in a timely manner.
  • Fax out physician orders for signature for those providers that are not staffed as active members.
  • Monitor the 30 day delinquent list.
  • Communicate with Medical Staff on providers who will be warned and suspended.
  • Mails or faxes documents to non-staff physicians within 30 days of discharge to assure record completion.
• Prepares and maintains an accurate delinquency list to ensure that only physicians having incomplete and overdue records are put on the lists utilizing the Cerner reporting system.
  • Delinquency letters are prepared and distributed with 98% accuracy weekly, per policy and procedures.
  • Suspension lists are sent to scheduling, medical staff, administration and management weekly; updates are sent daily, as needed.
• Notifies the physician office staff of number of delinquent medical records he/she has on a weekly basis. Documents on the worksheet the date and person notified according to the department procedure.
  • Documentation reflects physician called, number of delinquent records, and person spoken to.

  • Rounds to nursing units to pick up documents for auditing and scanning.
  • Assists with document auditing as time allows.
