This is a full-time day position Monday thru Friday
Job Summary:
The COVID-19 Care Team CHW uses their expertise about the community they are working with to provide education about COVID-19 and chronic disease prevention and treatment.
Essential Functions:
- Engage in community outreach and education about COVID-19 and chronic disease screening and health
- Learn and apply clinic policies and procedures to COVID-19 Care teamwork
- Educate community about chronic disease prevention and management
- Engage in relationship-building discussion with patients
- Log enrollments, educational activities, and referrals
- Track and follow-up with patients as part of clinical workflows, supporting team-based care
Position Qualifications:
- Have the ability to work independently with minimal supervision
- Possess the ability to encourage others to seek opportunities for different and innovative approaches to addressing problems and opportunities
- Great written and oral communication skills and be detail oriented
- Must have current driver’s license and reliable transportation
Skills & Abilities
Education-High school diploma or equivalent
- Complete Community Health Worker Certification Course within 6 months of being hired
- Complete Blood Pressure Self-Monitoring Program training and/or Diabetes Prevention Program lifestyle coach training as designated
