Community Health Worker/ Data Coordinator

The program will help to reduce the incidence and impact of Neonatal Abstinence Syndrome (NAS) in rural communities by improving systems of care, family supports, and social determinants of health. The target population for this grant is pregnant women, mothers, and women of childbearing age who have a history of, or who are at risk for, SUD/OUD and their children, families, and caregivers who reside in HRSA-designated rural areas.
JOB DUTIES

• Work collaboratively with internal and external partners to identify gaps in knowledge and services for women of childbearing age to reduce prevalence of Neonatal Abstinence Syndrome

• Coordinate community and professional education opportunities to increase knowledge and best-practices for preventing and treating Neonatal Abstinence Syndrome • Increase opportunities for the target population to access telehealth services for preventative and maternal care, treatment, and/or recovery services • Provide ongoing support and care coordination services as a CHW to women of childbearing age who have been diagnosed with Substance Use Disorder to enter and sustain recovery

QUALIFICATIONS/ REQUIREMENTS

• Bachelor’s degree preferred
• Preferred experience includes: community organizing, knowledge of the community, coordinating care, working with the public, and knowledge of the recovery community
• Flexibility and ability to travel within Coos and Northern Grafton Counties required
• Experience and comfort with Microsoft Office Suite, especially email, calendar scheduling, PowerPoint, and Excel
• Community Health Worker Training Certificate or willingness to be trained and achieve CHW Training Certificate
• Recovery Coach Training Certificate, or willingness to be trained and achieve Recovery Coach Training Certificate
