Nuvance Health has a network of convenient hospital and outpatient locations — Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York.
Summary:
This position will assist the Manager in the day-to-day support provided to caregivers located throughout the hospital, as well as to those who visit the Caregiver Center. This position will also support the volunteer Caregiver Coaches, providing guidance and assistance as needed. Responsible for coordination and execution of support groups and other programming as needed. Assist Manager in supporting professional caregivers as the situations arise.
Responsibilities:
1. Assess caregivers and their needs throughout the hospital as well as within the Caregiver Center. Offer guidance as appropriate, support through complex caregiving experiences, and provide stress reduction support as needed.
2. Collaborate with Hospitalists, Nurses, Doctors and the rest of the Healthcare Team relative to caregivers’ needs and provide recommendations for improved outcomes.
3. Have familiarity with interventions and support regarding transition, anticipatory grief and family systems dynamics.
4. Assist the Manager in training and overseeing the volunteer Caregiver Coaches, providing support and guidance regarding appropriate active listening skills, boundaries, cultural sensitivity, appropriate interactions with families, and self-reflection.
5. Assist with collecting statistical data regarding Center usage and supportive programming provided through the Caregiver Center.
6. Work well with interdisciplinary teams.
7. Collaborate with community-based organizations. Attend community-based programs as needed for the purpose of promotion and/or education regarding the Caregiver Center, caregiver needs, and helpful interactions.
8. Fulfills all compliance responsibilities related to the position.
9. Performs other duties as assigned.
