The Community Health Worker supports the resource needs of our community’s most vulnerable patients. This community facing position helps patients navigate social systems and barriers to care through referrals and direct services such as care coordination, delivering curriculum, Social Determinants of Health screening and assistance with resources and benefits.

Under supervision of the Health Advocacy Manager, the CHW works closely with providers, care teams, Regional Community Action Team (RCAT), and social services agencies to provide short term care coordination and connection to resources and support to program clients to improve their health and general well-being through education and provision of coordination of care and services. Works in both clinical and community-based settings, including clients’ homes. May frequently act as a clinical-facing CHW and provide support for the clinical-facing Community Health Advocate (CHA) team.
