Position Summary: This is NOT a remote position. Functions as a core member of the outreach team. The peer support specialist is responsible for providing and coordinating warm handoffs and mentoring to patients or potential patient families who are expecting or have children up to age two (2), who are impacted by SUD (substance use disorder) and in need of behavioral health services. The Peer Support Specialist (PSS) is expected to be a specialist and a generalist as he/she provides re-entry support in addition to providing experience-based input to guide the implementation of the strategic plan according to the community’s unique needs.
This position is 16 hours per week.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

• Maintains stable sobriety and be willing to self-identify publicly as being in recovery from substance use or addiction diagnosis.

• Obtains/maintains state certification for Peer Supporters.

• Understands the concept of recovery as it relates to behavioral health; peer support/recovery coaching; wellness and recovery planning.

• Facilitates patient engagement and follow-up care.

• Documents in-person and telephone encounters to report to Care Coordinator.

• Facilitates referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care).

• Participates in multi-disciplinary, team-based care of patients/clients.

• Participates in training, reporting, and other requirements of various grant supported programs implemented by BVHC and partners.

• Teaches one section of the Fatherhood is Sacred/Motherhood is Sacred per year.

• Offers Narcan training as often as needed for community groups and citizens.

MINIMUM JOB QUALIFICATIONS:

Education/Certificates/Licenses:

Required

• State certification for Peer Supporters (PSS 101 Completed)
• Valid driver’s license
• Master Narcan Trainer Certification (Must complete within 3 months of hire)

Preferred

• Certified Trainer for Fatherhood is Sacred/Motherhood is Sacred

Experience:

Required

• Lived experience with SUD
• Minimum two (2) years in recovery

Preferred

• Diverse Culture

WORKING CONDITIONS AND ENVIRONMENT:

• 16-hour workweek
• Evening and weekend hours allowed
• Working with challenging patients/clients
• Travel required to deliver Narcan and for monthly staff meetings
• Work from home with an option for office space if preferred
• Attend community events and be an active participant in community activities
