The Opportunity:
The Behavioral Health Care Manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s Primary Care Provider and other Mental Health Providers.

What you’ll be working on:
- Closely coordinating care with the patient’s Primary Care Provider and other members of the Mindset clinical team including Mental Health Providers
- Facilitating patient engagement, arranging follow-up care, and documenting patient progress and treatment recommendations in EMR and other required systems.
- Providing patient education about common mental health and substance abuse disorders and available treatment options, as well as recommending self-help resources as indicated
- Providing brief psychoeducational and skills training interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as directed by licensed clinician
- Facilitating treatment plan changes for patients who are not improving as expected in consultation with the patients team, as well as relapse prevention self-management plans with patients who have achieved their treatment goals and are soon to be discharged from the caseload
Education, licenses, and experiences required for this role:
- Masters Degree in one of the following: Social Work (MSW), Public of Health (MPH), Psychology, Counseling, Nursing
- At least 1 year of demonstrated abilities in case management and behavioral health care navigation and coordination
- Working knowledge of evidence-based psychosocial treatments and brief behavioral skills training interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
- Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role
- Experience with evidence-based coaching, counseling techniques or MH crisis line experience
- A working knowledge of common mental health and/or substance abuse disorders
- Ability to work during regular business hours M-F Pacific Time (required)

- The ability to effectively engage patients in a therapeutic relationship, when appropriate

This is a full-time, virtual role. Must reside in theUnited States.
One Medical is committed to fair and equitable compensation practices.
The base salary range for this role is $70,000 to $83,000 based on a full-time schedule. Final determination of starting pay may vary based on factors such as practice experience and patient care schedule. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. The total compensation package for this position may also include annual performance bonus, benefits and/or other applicable incentive compensation plans. For more information, visit
