1. Maintains a positive and enthusiastic attitude.
2. Assist individuals in daily living skills, personal hygiene and grooming as needed.
3. Works in cooperation with the departmental staff to identify specific medical appointments needed.
4. Schedules medical appointments and transports individuals to appointments.
5. Communicates all necessary information to RN, Program Services Manager, Team, and day services providers.
6. Prepares medication sheets in Administrative Office on monthly basis; updates as needed.
7. Updates medical calendars on the computer and in the calendar kept in the office.
8. Notify RN, QIDP, and Program Manager of any non-routine tests. visits, etc. and transports them back to group homes upon discharge, if applicable.
9. Maintains accurate records for consumer files.
10. Keeps necessary first aid supplies update in each of the homes
11. Maintains work environment in a safe, clean and orderly fashion.
12. Assists participants in Active Treatment as needed.
13. Requires lifting/ transferring of individuals to transport to and from medical appointments.
14. Plans and carries out social, recreational and community access activities with the individuals.
15. Presents individual with every opportunity to learn and develop their social behavior.
16. Observes and/or responsible for proper dispensation and charting of individual’s medications.
17. Follows all agency policies and procedures regarding the use and release of confidential information and confidential PHI (protected health information).
18. Adheres to all agency policies and procedures regarding ethics, client rights and dignity of persons served.
19. Must adhere to all necessary/required safety requirements.
20. Shall perform any other job-related or department-related function as assigned.
COMMUNICATION ANDKNOWLEDGE AND SKILLS
Strong communication skills (both oral and written)
Ability to independently organize time and duties to conduct the daily, weekly and monthly responsibilities of residential services MENTAL REQUIREMENTS
Work with minimal supervision.
Ability to handle multiple tasks and/or demands of one’s time from others.
Ability to work as a team member to facilitate service delivery.
Patience and diplomacy when dealing with difficult clients, staff, and other agencies.
Ability to handle stress.
EDUCCATIONAL AND EXPEREINCE REQUIREMENTS
High school diploma or GED equivalent
Proven experience working with mentally challenged individuals or those with other developmental disabilities.
Ability to safely support individuals through verbal, non-verbal and/or physical interventions.
Experience with handling confidential and sensitive information
PHYSICIAL REQUIRMENTS
Lifting between 25-50 lbs. Frequently, up to 150 lbs. occasionally.
Sitting to do paperwork, computer work and in meetings.

Participates in all scheduled medical trainings and in-service trainings
Required to change work and/or personal schedules to meet time requirements of position. May be asked to serve on various agency committees.
Valid driver’s license and proof of auto insurance is required.
Must have reliable car to transport participants to and from medical appointments.
