Compensation (Pay): $30.84 to $44.72 based on experience.

Job Overview:
The Psychiatric Nurse functions as a liaison between the patient and members of the Care Team in a joint effort to optimize the patients’ mental health status and quality of life.

The Psychiatric Registered Nurse will apply professional nursing care principles and procedures in the evaluation and treatment of psychiatric patients. Develop interpersonal therapeutic relationships with psychiatric patients, while following the guidelines set forth by the Colorado Nurse Practice Act.

Essential Duties and Responsibilities:

• Essential Duties and Responsibilities include the following.
• Participates as part of the interdisciplinary team to provide patients with appropriate services and ensures coordination of these services through the nursing process, to include assessment, planning, facilitation, complex focused care coordination, evaluation, advocacy and comprehensive care plan development to meet a patient’s complex needs.
• Functions within interdisciplinary team to support team-based care in assigned clinic, as well as other clinic locations.
• Assess high risk and rising risk patients consistent with the current Policies & Procedures for Care Management.
• Conducts patient visits to conduct intake assessments through interviews with patients and relatives to gather case history and medication list.
• Assessment for appropriate referrals.
• Plans and provides nursing care for patients-transition of care, nurse visits as indicated.
• Makes nursing assessments of the medical and emotional status of patients and recommends treatment options.
• Consults with psychiatrists and psychiatry nurse practitioner in designing appropriate treatment plans for patients.
• Visits patients and staff at the psychiatric hospital-promotes good relationships with staff from local facilities.
• Administers and notes reactions to psychotropic drugs (long-acting injections) and other medications. Arranges further medical attention when necessary.
• Assists in maintaining a safe and secure environment for staff and patients.
• Test for vital signs and record in EHR as needed.
• Maintains patient confidentiality.
• Maintains accurate records and documentation of patient treatments, immunizations, and laboratory results according to standard medical recordkeeping.
• Assess and triage per protocol for the appropriate level of care based on comprehensive risk assessment including diagnosis, clinical judgment, and outcome goals.
• Provides education to patients, families, care-givers.
• Encourages engagement and self-management of patients with regards to their own health including assisting the patient to understand the nature of emotional disturbances and accepting the need for treatment.
• Provides outreach to individuals, patients who are inactive in their healthcare, as well as the community by identifying and linking patients to available resources, social support systems, family support, etc.
• Provides in-reach to patients who are presently inactive in their healthcare.
• Facilitates access to quality and cost-effective care.
• Demonstrates an ability to utilize technology to include, but not be limited to, the Electronic Health Records, state registries, Health Information Exchange.
• Maintains documentation of all interactions accurately and completely, implementing appropriate protocols for patients’ charts and billing practices.
• Understands the immunization schedule and can support clinic staff and patients.
• Assists in the support of organizational clinic training needs for all clinical staff as assigned.
• Routinely attends trainings and meetings as recommended and required.
• Routinely participates in peer review, audits and chart audits as assigned.
• May be assigned to participate in precepting and training staff and students.

Qualifications:
Education/Experience:
Education: Graduate of an accredited Nursing Program required; BSN preferred. Possession of a valid and current Registered Nurse license issued by Colorado Board of Registered Nursing.

Work Experience: Three years’ experience in psychiatric nursing preferred or three years general nursing preferred with 1 year of psychiatry required.

Certificates and Licenses: Current Colorado RN License, Current CPR (BCLS) certification.

Physical Demands:
Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements and fine motor skills. Looking at computer screen continuously for long periods of time, hearing within normal range. Able to travel from clinic to clinic as required and make home visits as needed. Must be able to lift 20 pounds.
