OVERALL PURPOSE OF THE JOB
To willing assist the practice by seeing that the Medical Records Department has all charts ready for Doctors’ Schedules. This schedule should always be three days ahead. All forms filed into charts and help to assist all Medical Records Employees in any way to be sure that the Medical Records Department is ready for the next business day.
PRIMARY MEDICAL RECOR: DS DUUES
1. Retrieve medical record voice messages daily and complete record requests.
2. Take Dictaphones and download it to transcription on a daily basis, returning the Dictaphones to the physicians.
3. Retrieve the transcription and give to physicians to review and sign off.
4. When you receive the transcription back from physicians, it can be faxed to the referring physicians and file our copy back into the patient chart.
5. Search for charts of patients on schedule that day to assure that we have all patient charts by appointment time. If unable to locate these charts then notify the nurse and check in area.
6. Fax medical records that have been requested by another doctor’s office within 24hours. Test results requested by patient should be faxed once the doctor has signed that he has reviewed the report.
7. Put chart numbers on all incoming faxes and reports. Place the numbered faxes and reports in numbered order and put in the bin in chart room. Take out any reports and/or other forms with charts and place on Doctors desk for review and signature.
8. File all paperwork and forms daily making sure that all physicians have seen any important reports, lab results.
9. File back all charts from the “to be filed” area
10. Sort out prescriptions and give to triage with chart within 24hours of the fax.
11. Assist in answering the medical record phones to assure no more than three rings occur. Take appropriate messages using a message pad with date, time, date of birth, give chart or message to the nurse immediately.
12. Pull triage request off fax machine, pull charts and take to the triage room. This should be done anywhere between 5-15 minutes. If you can not find chart, it must be communicated to the physician, nurse and check in.
13. Pull triage charts for any areas that request inserting out guides to keep track of the charts.
14. Assist Front Desk with chart requests; assist patient with paperwork if you were ever requested.
15. Pull charts and paperwork and encounter form for all add en’s.
16. Print encounter forms and forms and give to check in at least by noon each day. Call and ask for any medical records that are needed. Leave a courtesy note if you have requested and not received the medical records from the patient’s medical office.
17. Retrieve charts from off-site storage facility (daily if needed).
18. Purge charts as needed. This will involve trips to the storage units to move and clean out as needed.
19. Make lab labels.
20. Remember Physicians will prefer to do their dictating at one time, please make sure you know the protocol.
