As a Patient Intake Specialist for Healthy Connect, your importance to our success cannot be overstated. You are the first contact a patient has with our brand after their Provider tells them about the service
The patients will have the program explained to them by their provider. The Healthy Connect Patient Intake Specialist meets with patients to educate them on the Healthy Connect Program and answer questions.
Duties and Responsibilities
- Correctly Demonstrate all the Medical Devices that will be issued as part of the program.
- Have an understanding of chronic conditions such as: High Blood Pressure, Diabetes, CHF, etc.
- Be able to complete the following tasks once training is completed:
- Determining the correct size of the blood pressure cuff
- Filling in and submitting a patient consent form
- Issuing devices from inventory using the RCP Portal
- Walk the patient through completing their first reading
- Welcome them to the program and schedule their first call with their assigned Nurse Coach
- Coordinate with office staff to determine and enroll eligible patients during their normal office visits.
- Connect with patients over the phone or in the office to provide care alongside the virtual nurse coach team.
- Support the culture of personalized patient care.
- Complete any required HIPAA training and/or certification
- Be professional in attitude and appearance when interacting with providers, practice staff and patients.
Requirements:
- LPN with at least 3 years of experience
- Office experience preferred
- EMR Experience
- Knowledgeable with using computers
- Self-starter mindset
Position Details:
- Full-Time
- Hourly – $22 – $25 / per hour
