Job Purpose

Perform adjustments and assignments of ICD-10-CM and CPT/HCPS based on clinical information. Comply with all legal requirements and insurance requirements regarding coding procedures and practices. Tasks include communication with providers for accurate documentation and documentation requirements.

Tasks

• Review the medical record to assure specificity of diagnoses and procedures for code assignment that is compliant, provides data, and optimizes reimbursement.
• Maintains current information on coding and compliance issues with relation to medical information resources to enable providers more accurate documentation.
• Abstracts information from medical records following established methods and procedures.
• Identifies problem documents and works with submitting departments to correct errors
• Verify codes and charges for accuracy.
• Track all charges and payments.
• Generate and follow up insurance and self-pay reports.
• Post and manage patient account payments.
• Investigate rejected claims and correct claims.
• Ensures clean claims and timely filing.
• Update billing forms to include revisions, deletions and additions ICD9/10 coding.
• Works towards compliance in all aspects of coding, participates in compliance activities as requested, and conducts in provider coding reviews.
• Collaborate with clinical staff and providers to assure accurate documentation to enhance data capture and optimal reimbursement for professional charges.
• Train providers and staff on medical coding.
• Liase with RCM staff and maintain remittance account enrolments
• Communicate with patients regarding account inquiries and due balances.
