POSITION SUMMARY
The Certified Nursing Assistant helps patients by supporting personal hygiene and daily living needs; providing comfort, transportation; and vital sign monitoring; assisting the staff nurses in the treatment and care of the patients in accordance with the established methods, protocols, policies and procedures of the hospital.  The CNAs responsibilities encompass compliance with The Joint Commission standards, CMS, OSHA, IDPH and state regulations.
POSITION SUMMARY

Position Duties and Responsibilities

1. Maintains and respects all patient rights and care for the patient in a dignified and compassionate manner.

Maintains and respects all patient related information (confidentiality).

2. Provides patients’ personal hygiene based on hospital policies, procedures and protocols at the discretion of and based on the judgment of a Registered Nurse by giving bedpans, urinals, commodes, baths, shampoos and shaves; assisting with travel to the bathroom; helping with showers.
3. Provides for activities of daily living by assisting with meals, feeding patients as necessary; ambulating, turning, and positioning patients; providing fresh water and nourishment between meals.
4. Provides adjunct care by administering enemas, nonsterile dressing, ice packs, heat treatments, sitz baths; applying restrains; discontinuing IV and urinary catheters.
5. Maintains patient stability by checking and documenting vital signs, daily weight, and intake and output.
6. Reports to a Registered Nurse any changes observed in the condition or behavior of the patient.
7. Completes patient care assignments in a timely manner and assists others as needed.
8. Makes rounds to patients, anticipates the patient’s needs, and answers/responds to the patient call-light or pages.
9. Transports patients to and from various areas as necessary.
10. Helps to prepare the unit, patient room, and patient bed for receiving patient admissions.
11. Assist in and as necessary, accompanies patients in the admission, transfer, and discharge process.
12. Handles and cares for the patient’s clothing and personal property (including prosthetic devices, hearing aids, dentures, glasses, contact lenses, etc.) from admission through discharge and documents patient belongings.
13. Collects, labels, and transports specimens to proper areas, as requested.
14. Assists in the cleaning and maintenance of patient rooms, utility rooms, supply and storage areas, and kitchen.
15. Ensures operation of equipment assigned to patient care by completing preventative maintenance requirements; following manufacturer’s instructions; troubleshooting malfunctions and reports problems and or orders services as needed.
16. Maintains supplies inventory by checking expiration dates and stock to determine inventory level; anticipating needed supplies; placing and expediting orders of supplies.
17. Uses supplies and patient care related items in a cost efficient manner.
18. Is pro-active in seeking out additional tasks when assignments are complete.
19. Attends at least 80% of all scheduled unit meetings held, and read the meeting minutes.
20. Assists in the orientation of new personnel to the unit.  Participates in the peer interview process.
21. Supports the philosophy, goals and objectives of the Nursing Department.
22. Accountable for own conduct and promotes good working relationships among staff and between other disciplines and departments.
23. Assumes other duties as assigned within scope of practice.

POSITION QUALIFICATIONS
Education

1. Must be a high-school graduate or successfully completed the GED. 
2. Must have completed a Nursing Assistant Program and be certified in the state of Illinois.
3. Must complete a Basic Dysrhythmia class annually.

Experience          

1. Must be currently CPR certified. 
2. Previous Medical/Surgical experience desirable.
3. Must show competency of those skills designated by the Manager of Inpatient Services by written exam and/or demonstration. 
4. Must be in good physical and mental health, and able to work under pressure. 
5. Must have good communication skills.
6. The ability to operate designated computer equipment.

Work Environment/Physical Demands

1. Requires long periods of standing at times.
2. Requires quick movement, agility and fast response time.
3. Requires direct patient contact with potential exposure to blood and body fluids.
4. Frequent lifting and/or handling of light weight items.
5. Occasionally lifting up to 35 pounds or more with an assistive device/and or physical assistance.

POSITION SUMMARY
The Health Unit Coordinator performs the receptionist and secretarial duties of various patient care units.  His/her primary responsibility is to coordinate patient care across many functional departments.  The Health Unit Coordinator is proficient at reading basic single lead telemetry monitoring and can identify basic life threatening arrhythmias.   The Health Unit Coordinator works effectively and harmoniously with patients, personnel, and others.  The Health Unit Coordinator demonstrates integrity, dependability, honesty, resourcefulness, and enthusiasm for work.
Position Duties and Responsibilities

1. Receives, greets, and directs patients, visitors, and hospital personnel arriving on or calling patient care units in a professional manner.
2. Acts as a focus of communication regarding all patient care activities, patient location, and able to provide information and direction regarding the same, with regard to patient’s age group and while maintaining patient confidentiality.
3. Receives and communicates requests from patients, families, and physicians, and refers requests to nursing staff or appropriate hospital personnel.
4. Observes hospital and unit policy in assisting and directing patients, staff and visitors.
5. Practices pleasant phone manners when dealing with patients, families, and hospital staff.
6. Observes patient telemetry for alarms and notifies primary care nurse immediately for interpretation of rhythms.
7. May function as the recorder during CODE BLUE emergency situations.
8. Facilitates stat requests and orders.
9. Records and delivers messages for patients, physicians and staff.
10. Is computer competent and is able to perform the necessary skills needed to complete orders, and obtaining medical information for the patient’s medical record.  Is able to troubleshoot system problems should they arise.
11. Receives and distributes mail, flowers, and various other items for patients on the unit.
12. Maintains adequate quantity of forms, materials, and equipment inventory on assigned units.
13. Maintains a clean, safe, and functional working environment in the nursing station.
14. Notifies dietician of consultation requests and needs based on admission dietary evaluation protocol.
15. Notifies kitchen of any diet changes due to x-ray, change in physician order, etc., via the computer.
16. Initiates, assembles and maintains charts for all patients on the assigned units.
17. Transcribes physician’s orders; enters all necessary commands via the computer needed to facilitate and implement orders.  Communicates information to the appropriate personnel.
18. Initiates and updates information on the patient Kardex.
19. Files diagnostic and laboratory reports after nursing review in appropriate patient charts.
20. Maintains the patient chart in appropriate, functional order as prescribed by hospital and unit policy.  Monitors location of charts at all times and prepares transfer of chart to Health Information Management upon discharge.
21. Communicates all pertinent patient care information and general environmental information to the Manager of Medical-Surgical Nursing, Supervisor, and other unit staff.
22. Is pro-active in attending in-service education programs both in and out of house and keeps educational records current.
23. Maintains mandatory educational requirements based on JCAHO, IDPH, and OSHA Standards.
24. Is responsible and accountable to the Manager of Medical-Surgical/Intensive Care & Outpatient Infusion Services for maintaining current BLS certification as required.
25. Assists in the orientation of new personnel to the unit.
26. Uses supplies and patient care items cost effectively.
27. Maintains computer skills and ability to assist other hospital personnel with computer use.  Utilizes the computer to make communication with other departments and staff as easy as possible.
28. Assists on the patient care unit when needed and able to leave the nurse’s station, i.e., passing trays, answering lights, etc.  Is able to anticipate and answer patients needs based on age.
29. Shall attend at least 90% of all scheduled unit meetings held, and read the meeting minutes.
30. Supports the philosophy, goals and objectives of the Nursing Department.
31. Strives for quality in patient care and as necessary challenges and/or corrects errors of peers to ensure patient safety and well being.

POSITION QUALIFICATIONS

Education

1. Must be a high-school graduate or successfully completed the GED.

 
Experience          

1. Previous experience in clerical or receptionist position helpful. 
2. Hospital experience or satisfactory completion of hospital training program preferred. 
3. Must be CPR certified and basic EKG/telemetry monitor competent or willingness to pursue. 
4. Must attend annually a basic dysrhythmia class for maintaining basic rhythm interpretation.
5. Must be in good physical and mental health, and able to work under pressure. 
6. Must have good communication skills.
7. The ability to operate designated computer equipment.
